阻塞性睡眠呼吸暂停低通气综合征患者超敏血清C反应蛋白和T淋巴细胞亚群水平的变化及临床意义

The change and clinical significance of T lymphocyte subsets and serum hypersensitive-C reactive protein levels in patients with obstructive sleep apnea hypopnea syndrome

目的评价阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者血清超敏C反应蛋白(hypersensitive-C reactive protein,hs-CRP)和T淋巴细胞亚群(T lymphocyte subsets,T-LS)的变化,探讨OSAHS对T-LS水平的影响及hs-CRP水平、T-LS水平与OSAHS之间的相互关系。方法选择门诊及住院的单纯OSAHS患者59例,同期健康体检者24例作为对照组,两组之间年龄和性别等构成差异均无统计学意义。所有受试者均接受连续7 h的夜间多导睡眠仪监测以判断OSAHS患者病情的严重程度;于次日清晨采集外周静脉血5 mL,测定血清中hs-CRP和T-LS水平。结果 OSAHS组CD3^+CD4^+T细胞较对照组明显降低,差异有统计学意义(P<0.05);而CD3^+、CD3^+CD8^+T细胞和CD4^+/CD8^+T细胞较对照组差异均无明显统计学意义(P>0.05)。结论 OSAHS患者体内存在T-LS水平的改变,可能导致不同程度的免疫功能下降。

Objective To evaluate the change of T lymphocyte subsets and serum hypersensitive-C reactive protein（hs-CRP）level in patients obstructive sleep apnea hypopnea syndrome（OSAHS）,and to explore OSAHS′s influence on the level of T lymphocyte subsets（T-LS）and the relationship among hs-CRP,level of T lymphocyte subsets and OSAHS. Methods A total of 59 in-patients and outpatients with OSAHS was selected as the OSAHS group,24 healthy subjects were selected as control group.There was no significant differences in gender and age between these two groups.All subjects underwent 7 hours continuous nighttime polysomnography（PSG） monitoring to access their disease severity of OSAHS.The next morning,5 mL blood was collected from peripheral vein for testing the levels of hs-CRP and T-LS. Results Compared with control group,the level of CD3^＋CD4^＋T lymphocyte in OSAHS group was decreased with statistical significance,but the levels of CD3^＋,CD3^＋CD8^＋and CD4^＋/CD8^＋T lymphocyte had no significant difference（P〉0.05）.Conclusions Changes of T-LS level in OSAHS patients may lead to decline of immunologic function in different levels.
