痰热清联合舒利迭治疗慢性阻塞性肺疾病

Tanreqing in combined with seretide in treatment of chronic obstructive pulmonary disease

目的探讨痰热清联合舒利迭治疗慢性阻塞性肺疾病(COPD)临床疗效。方法选取70例COPD患者为研究对象,随机分为治疗组和对照组,各35例,2组均给予止咳、祛痰、平喘、抗感染、吸氧、纠正水电解质及酸碱平衡等常规治疗,并采用沙美特罗/替卡松粉吸入剂(舒利迭)雾化吸入,1吸/次,2次/d;治疗组在上述治疗基础上加用痰热清20 m L加5%葡萄糖注射液或0.9氯化钠注射液250 m L静脉点滴,1次/d,均10 d为1个疗程。对比2组治疗前后血气指标变化情况、肺功能改善情况及临床疗效。结果治疗后Pa O2、Pa CO2、Sa O2、p H值均较治疗前改善(P<0.05),治疗后Pa O2、Pa CO2、Sa O2改善情况治疗组优于对照组(P<0.05);治疗后,FEV1、FEV1/FVC、FEV1%预测值均较治疗前升高(P<0.05),治疗组升高程度优于对照组(P<0.05);对照组总有效率为71.4%,治疗组为94.3%(P<0.05)。结论痰热清联合舒利迭治疗COPD可以显著改善患者血气指标,改善肺功能,提高临床疗效。

Objective To explore the clinical efficacy of Tanreqing in combined with seretide in the treatment of chronic obstructive pulmonary disease（ COPD）. Methods A total of 70 patients with COPDwere included in the study and randomized into the observation group and the control group. All the patients were given cough relieving,phlegm eliminating,asthma relieving,anti-infection,oxygen inhalation,water-electrolyte and acid base balance correcting these routine treatments,and aerosol inhalation of salmeterol / flonase（ seretide）,once a time,twice a day. Based on the above treatments,the patients in the observation group were given intraveneous drip of Tanreqing（ 20 m L） ＋ 5% glucose injection or 0. 9% sodium chloride injection（ 250 m L）,once a day. Ten day treatment was regarded as one course. The change of blood gas indexes,pulmonary function improvement,and the clinical efficacy before and after treatment in the two groups were observed and compared. Results The improvement of Pa O2,Pa CO2,Sa O2,and p H after treatment was significantly superior to that before treatment（ P〈0. 05）. The improvement of Pa O2,Pa CO2 and Sa O2 after treatment in the observation group was significantly superior to that in the control group（ P〈0. 05）. The predicative values of FEV1,FEV1 / FVC,and FEV1% after treatment were significantly higher than those before treatment（ P〈0. 05）,and the increased degree in the observation group was significantly superior to that in the control group（ P〈0. 05）. The total effective rate in the observation group（ 94. 3%） was significantly higher than that in the control group（ 71. 4%）. Conclusion Tanreqing in combined with seretide in the treatment of COPD can significantly improve the blood gas indexes and pulmonary function,and enhance the clinical efficacy; therefore,it deserves to be widely recommended in the clinic.
