64排螺旋CT诊断冠状动脉狭窄病变患者的临床应用价值研究 被引量:14

摘要 目的:探讨64排螺旋CT在冠状动脉狭窄病变患者中的诊断价值。方法:以本院心血管内科收治的经临床诊断或疑似冠状动脉狭窄病变的124例患者为研究对象,分别对其进行64排螺旋CT冠状动脉成像诊断和选择性冠状动脉造影检查,以冠状动脉造影检查结果为金标准,对比分析两种诊断方式的诊断结果。结果:64排螺旋CT冠状动脉成像对冠状动脉狭窄情况(通畅、轻度狭窄、中度狭窄、重度狭窄、闭塞)与冠状动脉造影相比,差异无统计学意义(P>0.05)。64排螺旋CT冠状动脉成像诊断冠状动脉狭窄的敏感度为98.4%,阳性率为63.7%,钙化病变的敏感度为96.0%,阳性率为61.2%。结论:64排螺旋CT冠脉动脉成像对冠状动脉狭窄病变的诊断具有重要的临床价值,其影像具有一定的特征性,对狭窄的诊断能力比较强,诊断准确率高,具有无创检查、定位准确的优势,是诊断冠心病的理想检查方式,值得临床推广使用。 Objective: To investigate diagnostic value of 64-slice spiral CT on patients with coronary artery stenosis disease. Method: 124 patients suspected with coronary artery stenosis lesions via clinical diagnosis were selected as research subjects, and retrospective analysis and summary were made according to their clinical data. 64-slice spiral CT coronary artery imaging diagnosis and selective coronary angiography examination were respectively conducted, coronary angiography result was treated as the gold standard, and diagnostic results of diagnosis ways were compared and analyzed. Result: The differences in manifestations of 64-slice spiral CT coronary artery imaging on conditions of coronary artery stenosis ( patency, mild stenosis, moderate stenosis, severe stenosis and occlusion ) showed no differences when compared to coronary angiography ( P〉0.05 ) . The sensitivity and positive rat of 64-slice spiral CT coronary artery imaging in the diagnosis of coronary artery stenosis was 98.4% and 63.7%, and indexe of calcified lesions was 96.0% and 61.2% respectively. Conclusion: 64-slice spiral CT coronary artery imaging delivers important clinical value in the diagnosis of coronary artery stenosis disease, the image throws certain characteristics, it is provided with advantages including powerful diagnosis ability of stenosis, high diagnostic accuracy, non-invasive examination and accurate positioning, being ideal examination means in the diagnosis of coronary heart disease, and being worthy of clinical service.
