大承气汤保留灌肠对剖宫产术后肛门排气和胃肠功能恢复的影响

Effects of Retention Enema with Dachengqi Decoction on the Anal Exsufflation and Recovery of Gastrointestinal Function after Cesarean Section

目的:观察大承气汤保留灌肠对剖宫产术后首次肛门排气时间、首次排便时间和肠鸣音恢复时间的影响,探讨大承气汤对剖宫产术后患者胃肠功能恢复的作用。方法:选择采用硬膜外麻醉行剖宫产术的患者138例,随机分为治疗组和对照组各69例。对照组于剖宫产术后6 h开始采用开塞露20 m L塞肛,治疗组采用大承气汤进行保留灌肠,术后12 h起,每24 h保留灌肠1次,患者排便后停止灌肠。观察两组产妇返回病房至首次排气、首次排便的时间、肠鸣音恢复正常的时间。结果:治疗组首次排气时间、首次排便时间、肠鸣音恢复正常时间均短于对照组,差异均有统计学意义(P<0.01)。治疗组术后18 h、24 h肠鸣音评分优于对照组,差异均有统计学意义(P<0.01)。治疗组治愈率为89.9%,对照组为59.4%,两组比较,差异有统计学意义(P<0.01)。结论:大承气汤保留灌肠可缩短剖宫术后患者肠鸣音恢复时间、术后首次排气时间、首次排便时间,促进胃肠功能的恢复。

Objective： To observe the effects of retention enema with Dachengqi decoction on the first anal exsufflation time, the first defecation time, the recovery time of gurgling sound after cesarean section, and to study the effects of Dachengqi decoction on the recovery of gastrointestinal function after cesarean section. Methods： The 138 patients after cesarean section were randomly divided into two groups： the treatment group （n=69 cases） and the control group （n=69 cases）. The patients in control group were treated with 20 mL glycerin suppository through rectum 6 hours after cesarean section, while the treatment group were treated with Dachengqi decoction retention enema 12 hours after cesarean section, 1 time/24 h, until the first defecation time. Results： The first anal exsufflation time, first defecation time, gurgling sound recovery time of patients in treatment group were shorter than those of patients in contorl grous with statistically significant obviously （P〈0.01）. The gurgling sound scores of patients in treatment were better than control group at 18 h and 24 h after cesarean section （P〈 0.01）. The cure rate of treatment group was 89.9%, and the control group was 59.4%, there was no statistically significant difference （P〈0.01）. Conclusion： Retention enema with Dachengqi decoction can shorten the anal exsufflation time, first defecation time, recovery time of gurgling sound, and improve the recovery of gastrointestinal function after cesarean section.
