外侧楔形截骨矫正儿童肘内翻畸形手术中内固定方式的选择 被引量:3

摘要 目的:探讨外侧楔形截骨矫正儿童肘内翻畸形手术中不同内固定方式的临床疗效和安全性。方法:2005年6月至2013年8月收治73例(78肘)肘内翻畸形患儿。根据术前在肘关节伸直正位X线片上测量计算的矫正截骨角度(肘内翻角+正常提携角)进行肱骨外侧楔形截骨。截骨完成后,对于年龄较小、术前肘关节活动度较好的患者,采用交叉克氏针内固定或克氏针钢丝张力带内固定;对于年龄较大、病程较长、肘关节活动度差甚至僵硬或肱骨髁部畸形的患者,采用钢板内固定。观察记录患者的手术时间、截骨愈合情况,拆除内固定后3个月评价患侧肘关节活动度,并采用Jupiter等制定的肘关节功能评分标准评定肘关节功能。结果:所有患者均顺利完成肱骨外侧楔形截骨术,其中27例(30肘)行交叉克氏针内固定(交叉克氏针组)、21例(21肘)行克氏针钢丝张力带内固定(克氏针张力带组)、25例(27肘)行钢板内固定(钢板组)。术后所有患者均获随访,随访时间12~60个月,中位数18个月。3组患者截骨处均获骨性愈合。内固定拆除后3个月,交叉克氏针组、克氏针张力带组和钢板组患者的肘关节活动度分别为132.4°±10.2°,132.5°±9.5°,131.9°±6.8°;按照Jupiter肘关节功能评分标准评定肘关节功能,交叉克氏针组优18肘、良9肘、中3肘,克氏针张力带组优13肘、良6肘、中2肘,钢板组优16肘、良8肘、中3肘。至随访结束时,3组各有1肘发生尺神经损伤,交叉克氏针组2肘出现内固定松动、断裂,克氏针张力带组1肘出现内固定松动、断裂。结论:采用肱骨外侧楔形截骨术矫正儿童肘内翻畸形时,根据患儿的年龄、病程及术前肘关节活动度分别选择交叉克氏针内固定、克氏针钢丝张力带内固定或钢板内固定,术后肱骨截骨处愈合好,肘关节功能恢复满意,并发症少。
