摘要
目的:分析骨髓细胞形态学(M)、细胞免疫学(I)、细胞遗传学(C)和分子生物学(M)联合方法(MICM分型)在白血病诊断中的价值。方法:收集196例白血病患者骨髓细胞形态学、细胞免疫学、细胞遗传学及分子生物学检查结果,比较分析单一的形态学分型、单一细胞免疫学分型、形态学联合细胞免疫学分型与MICM分型4种检查方法对白血病诊断的符合率。结果:196例患者,单一形态学分型、单一细胞免疫学分型、形态学联合细胞免疫学分型与MICM分型4种方法对白血病的诊断符合例数分别为158、159、182例、196例,符合率分别为80.6%、81.1%、92.8%、100%;形态学联合细胞免疫学分型比单一细胞形态学或单一细胞免疫学分型诊断符合率高,差异有统计学意义(P<0.01);MICM分型比形态学联合细胞免疫学分型诊断符合率高,差异有统计学意义(P<0.01)。结论:MICM分型可提高白血病的诊断率。
Objective: To explore the clinical value of MICM typing in the diagnosis of leukemia. Methods: 196 cases of patients with diagnosed leukemia were enrolled in this study, and the experimental results of cell morphology, cellular immunology, eytogeneties and molecular biology of their bone marrow cell were collected. 4 examination methods including single cell morphology typing, single cellular immunology typing, cell morphology combined with cellular immunology typing and MICM typing were compared for diagnosis accuracy of leukemia. Results: In 196 cases of patients with leukemia, the diagnostic accordance rate of single cell morphology typing was 80.6% (158/196) , the diagnostic accordance rate of single cellular immunology typing was 81.1% (159/196) , the diagnostic accordance rate of cell morphology combined with cellular immunology typing was 92.8% (182/196) and the diagnostic accordanee rate of MICM typing was 100% (196/196). The diagnostic aeeordanee rate of eell morphology combined with cellular immunology typing was signifieantly higher than that of single cell morphology typing or single cellular cordance rate of MICM typing was significantly immunology typing (P 〈 0.01 ) while the diagnostic achigher than that of cell morphology combined with cellular immunology typing (P 〈0.01 ). Conclusion: MICM typing can improve the diagnostic rate of leukemia.
