ICU患者预防非计划性拔管集束化护理研究

Research of ICU Patients' Unplanned Extubation Cluster Nursing

目的:研究ICU患者预防非计划性拔管的集束化护理措施。方法:选取2013年2月-2014年10月本院收治的ICU患者360例,按照入院顺序分为观察组和对照组各180例。对照组采取常规护理干预措施,观察组患者采取集束化护理干预措施。对所有患者的非计划性拔管事件进行记录,并对非计划性拔管导致的并发症或者死亡情况进行统计。通过问卷调查的方式对两组患者的满意度进行调查,两组患者的责任护士分别进行自我评价。结果:观察组患者的非计划性拔管率(2.22%)显著低于对照组(6.70%),比较差异具有统计学意义(P<0.05)。观察组患者因非计划性拔管导致1例并发症,对照组有6例患者出现并发症,其中2例死亡,两组并发症发生率差异具有统计学意义(P<0.05)。观察组患者的满意度(97.22%)显著高于对照组(88.33%);观察组责任护士对工作的自我满意度评分(96.25±3.8)分及专业知识掌握程度评分(95.86±3.6)分均显著高于对照组,比较差异具有统计学意义(P<0.05)。结论:对ICU患者实施集束化护理,有效降低了非计划性拔管事件的发生率,其效果良好,值得广泛推广使用。

Objective： To study ICU patients＇ unplanned extubation cluster nursing. Method： 360 ICU patients from February 2013 to October 2014 in our hospital were selected and divided into observation group and control group （ each group contained 180 eases ） according to the order of admission. The control group implemented routine care interventions while the observation group was taken cluster nurs＇ing interventions. All the patients＇ unplanned extubation events and complications or deaths were recorded. Patient satisfaction between the two groups was surveyed through questionnaires, and the self-evaluation of the two groups＇ duty nurses as well. Result： The patients＇ unplanned extubation rate of the observation group was 2.22%, which was significantly lower than 6.70% of the control group, the difference was statistically significant （ P〈0.05 ） . 1 patient in the observation group resulted complications due to unplanned extubation, while in the corrtrol group, 6 patients had the same problem and 2 cases died. There was statistically significant difference between the two groups （ P〈0.05 ） . Patient satisfaction of the observation group was 97.22%, which was obviously higher than 88.33% of the control group, the difference was statistically significant （ P〈0.05 ） . The self-satisfaction on jobs of nurses of the observation group scored （ 96.25 ± 3.8 ） points, as well as the mastery of professional knowledge scored （ 95.86 ± 3.6 ） points, which was obviously higher than that of the control group, the difference was statistically significant. Conclusion： The implementation of cluster bundled nursing, on ICU patients effectively reduces the incidence of unplanned extubation event, brings good effect and worthy of widely used.
