胃癌手术患者的临床病理特点研究

The Research of Clinical Pathological Features of Gastrectomy Patients

目的：探讨不同部位胃癌手术患者的临床病理特点。方法：选取2012年1月-2014年12月于本院普通外科进行根治性胃癌切除手术的110例胃癌患者作为研究对象,并根据肿瘤部位的不同分为胃底贲门部（U组）、胃体小弯部（M组）和胃窦幽门部（L组）,从性别、年龄、癌肿大小、病理分型和TNM分期等方面对不同部位胃癌患者的临床病理特点进行回顾性分析与总结。结果：110例患者中,U组16例,占病例总数的14.55%;M组28例,占病例总数的25.45%;L组66例,占病例总数的60.00%。U组男女比例为4.33∶1（13/3）,男性所占比例最大,平均年龄（60.23±10.25）岁,三组中发病年龄最高（P〈0.05）。癌肿大小方面,U组最大,L组最小（P〈0.05）。三组胃癌Borrmann分型比较,U组浸润型（Ⅲ~Ⅳ型）所占比例最大,L组最小（P〈0.05）。结论：不同部位的胃癌有其较为独立的临床病理特点,胃体小弯部肿瘤其临床病理特点介于胃底贲门部和胃窦幽门部,所以在治疗时,应该结合其临床特点,具体问题具体分析。

Objective： To investigate the clinical pathological features of gastric carcinoma surgery with different tumour locations. Method： 110 gastric cancer patients received radical gastrectomy treatment in our hospital from January 2012 to December 2014 were chosen as the research objects. According to the different locations of the tumor, patients were divided into gastric cardia （ group U ）, gastric body lesser curvature （ group M ） and gastric antrum pylorus （ group L ） . The clinical pathological features of the three groups were compared in gender, age, tumor size, pathological type, and TNM staging. Result：Among the 110 cases, the group U contained 16 case （ 14.55% ）, the group M contained （ 28 cases ）, the group L contained 66 cases （ 60.00% ） . The ratio of male patients with female in the group U arrived 4.33 ： 1 （ 13/3 ）, in which the average age was （ 6.023 ± 10.25 ） years old, showed as the 1artiest male proportion and the eldest age of onset （ P〈0.05 ） . The tumor size of the group U was the maximum and size of the group L was the minimum （ P〈0.05 ） . The proportion of infiltration type （ Ⅲ-Ⅳ ） of the group U was the largest, which in the group L was the minimum （ P〈0.05 ） . Conclusion： Different locations of gastric cancer has its relatively independent of the clinical pathological features. The pathological features of gastric body lesser curvature cancer is between gastric fundus cardia and antrum pylorus. Therefore, the gastric treatment must on the basis of the clinical characteristics and specific issues analysis respectively.
