综合护理降低尿毒症患者血透并发症发生率的作用分析

Effect of comprehensive nursing on the incidence of complication in patients with uremic hemodialysis

目的探讨综合护理降低尿毒症患者血透并发症发生率的作用。方法采用回顾性分析法,选取我院2014年3月至2015年6月期间收治的120例尿毒症患者临床资料,按照护理方法的不同将患者分为2组,即对照组和观察组,每组各60例。对照组采用血液透析常规护理措施,干预组在对照组常规护理基础上实施综合护理并进行辅助治疗。观察2组患者低血压、出血、猝死、心率失常等血液透析并发症发生率和护理效果。结果经过一系列的护理,发现观察组患者有10例低血压、1例出血、6例心率失常,并发症发生率为28.33%;对照组患者中有25例低血压、2例出血、1例猝死、12例心率失常,并发症发生率为66.67%。可见观察组患者并发症发生率明显低于对照组,差异具有统计学意义(P<0.05);观察组患者护理满意度为98.30%,明显高于对照组73.33%的护理满意度,差异具有统计学意义(P<0.05)。结论综合护理干预对尿毒症患者具有较好的临床效果,不仅能够有效降低患者血液透析并发症发生率,而且能够有效提高患者护理满意度,促进患者康复,值得在临床上应用推广。

Objective To investigate the effect of comprehensive nursing on the incidence of complication in patients with uremic hemodialysis. Methods Retrospective analysis was made on the clinical data of 120 patients with uremia admitted into our hospital from March 2014 to June 2015. In accordance with different nursing methods, the patients were divided into two groups, the control group and the intervention group, each consisting of 60 patients. The control group received routine nursing care, while the intervention group was given comprehensive nursing and auxiliary treatment, in addition to routine nursing care. Close observations were made on the incidence of such complications as hypotension, hemorrhage, sudden death, arrhythmia induced by hemodialysis, and the nursing effect as well. Results After a series of nursing interventions, there occurred 10 cases of hypotension, 1 case of hemorrhage, 6 cases of arrhythmia, with 28.33% incidence rate of complications in the intervention group, while on the other hand, there occurred in the control group 25 cases of hypotension, 2 cases of hemorrhage, 1 case of sudden death, 12 cases of arrhythmia, with 66.67% incidence rate of complications. Detection resuhs revealed that the incidence rate of complications for the patients of the intervention group was obviously lower than that of the control group, and there was statistical significance, when comparisons were made between the 2 groups （ P 〈 0.05 ）. The rate of satisfaction with nursing care for the patients of the intervention group was 98.33% , which was obviously higher than that of the control group （73.33%） , and there was also statistical significance, when comparisons were made between the 2 groups （ P 〈 0.05 ）. Conclusion Comprehensive nursing intervention produced better clinical effect on the patients with uremia, which could not only effectively reduce the incidence rate of complications in patients with uremic hemodialysis, but effectively increase the rate of satisfaction with nursing care and promote patient recovery. For this reason, it isworth further clinical extension.
