摘要
目的探讨超声引导下粗针穿刺活检(US-CNB)对甲状腺超声影像报告和数据系统(TI-RADS)4类甲状腺结节的诊断价值。方法分析甲状腺患者的超声特征,根据TI-RADS对甲状腺结节的恶性风险进行评估,对177例患者TI-RADS 4a、4b和4c类共189个结节结节均行US-CNB检查,其组织学结果与术后病理结果进行对照研究。结果 US-CNB组织学诊断与术后病理结果差别无统计学意义(P>0.05);TI-RADS 4类甲状腺结节的灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为95.7%,97.8%,97.8%,95.7%和96.7%。结论US-CNB检查对TI-RADS 4类甲状腺结节有较高的诊断价值,能有效提高术前诊断的准确性。
Objective To evaluate the diagnostic value of ultrasound-guided core-needle hiopsy (US-CNB) for thyroid imaging reporting and data system (TI-RADS) category 4 thyroid nodules. Methods 189 thyroid nodule of ultrasound features in 177 patients were retrospectively analyzed. The malignant risk of every nodule were evaluated and classified by TI-RADS. A total of 189 thyroid nodules of TI-RADS category 4a 4b and 4c were underwent US-CNB . The histopathology results of US-CNB and pathological results of thyroid surgery of nodules were used to compared. Results There was no significant differences between results of US-CNB and pathological results of thyroid surgery in thyroid nodules (P〉0. 05). The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of thyroid nodules with US-CNB in TI-RADS category 4 were 95.7% ,97.8% ,97.8% ,95.70% and 96.7% respectively. Conclusion There was higher diagnostic value by US-CNB for the TI-RADS category 4 thyroid nodules. US-CNB can effectively improves the accuracy of thvroid nodules.
