降钙素原联合病原体检测在下呼吸道感染诊断中的应用观察

Observation of application of procalcitonin combined with pathogen detection in diagnosis of lower respiratory tract infection

目的探讨降钙素原联合病原体检测在下呼吸道感染诊断中的应用效果。方法 2012年12月~2014年12月期间,随机抽取87例下呼吸道感染患者作为研究组,再按临床诊断分为研究Ⅰ组（细菌感染45例）与研究Ⅱ组（非细菌感染42例）,同期选择41例健康体检者作为对照组,所有研究组患者均进行血清降钙素原（PCT）和血清抗体（Ig M）检测,且进行痰培养,对照组仅进行血清降钙素原检查,各组间数据进行相关统计学对比分析。结果研究Ⅰ组PCT水平明显高于研究Ⅱ组与对照组[（5.08±0.79）ng/m L〉（0.52±0.20）ng/m L〉（0.14±0.03）ng/m L],差异有统计学意义（P〈0.05）。研究Ⅰ组PCT阳性检出率、痰培养阳性检出率、病原体Ig M抗体阳性检出率（91.11%、31.11%、4.88%）,与研究Ⅱ组相比（23.81%、0、52.38%）,差异有统计学意义（P〈0.05）。结论降钙素原联合病原体检测在下呼吸道感染诊断中,有重要临床指导意义。

Objective To explore applicative effect of procalcitonin combined with pathogen detection in diagnosis of lower respiratory tract infection. Methods 87 patients with lower respiratory tract infection from December 2012 to December 2014 were randomly selected and they were allocated to the research Ⅰ group（45 cases in bacterial infection group）and the research Ⅱ group（42 cases in non-bacterial infection group） according to clinical diagnosis.And 41 healthy cases in the same period were selected as the control group. Serum procalcitonin（PCT） and serum antibody（Ig M） of patients in two research groups were detected and received sputum culture.The control group was only received serum procalcitonin detection. Statistics of three groups were received correlation statistical comparison and analysis. Results PCT level of the research Ⅰ group was（5.08±0.79）ng/m L,significantly higher than that of the research Ⅱ group（0.52±0.20）ng/m L and the control group（0.14±0.03）ng/m L. Differences had statistical significance when compared（P〈0.05）.Positive rates of PCT,sputum culture and pathogen Ig M antibody of the research Ⅰ group and the research Ⅱ group were（91.11%,31.11%,4.88%）and（23.81%,0,52.38%）respectively.Differences had significant statistical significance（P〈0.05）. Conclusion Procalcitonin combined with pathogen detection in diagnosis of lower respiratory tract infection has important clinical guiding significance.
