甲状腺影像学报告及数据系统联合超声弹性成像对甲状腺结节的诊断价值

Value of thyroid imaging report and data system combined with ultrasound elasticity imaging in diagnosis of thyroid nodules

目的探讨甲状腺影像学报告及数据系统（TI-RADS）联合超声弹性成像（UE）对甲状腺结节的诊断价值。方法选取2012年1月~2014年5月在我院诊治的甲状腺结节患者100例（160个结节）,给予常规超声及UE检查,并经病理证实。比较TI-RADS与UE对甲状腺结节良、恶性的鉴别诊断价值。结果病理结果示,160个甲状腺结节中有良性122个,恶性38个。UE的ROC曲线下面积（AUC）、敏感度、特异度、准确性、阴性预测值及阳性预测值均明显高于TI-RADS,差异有统计学意义（P〈0.05）。TI-RADS联合UE的AUC、敏感度、特异度、准确性、阴性预测值及阳性预测值则明显高于单一UE及TI-RADS方法 ,差异有统计学意义（P〈0.05）。结论 TI-RADS、UE对甲状腺良、恶性结节有一定的鉴别诊断作用,但以UE的诊断价值更高,而联合应用两种方法可提高诊断甲状腺结节良、恶性的准确性。

Objective To explore value of thyroid imaging report and data system（TI-RADS）combined with ultrasound elasticity imaging（UE） in diagnosis of thyroid nodules. Methods 100 patients with thyroid nodules（160 thyroid nodules） who were admitted to our hospital from January 2012 to May 2014 were selected. They were received routine ultrasound and UE examination. In addition,thyroid nodules were confirmed by pathology. Differentiation and diagnostic values of TI-RADS and UE for benign thyroid nodules and malignant thyroid nodules were compared. Results Pathological results showed that there were 122 benign thyroid nodules and 38 malignant thyroid nodules. ROC area under curve（AUC） sensitivity, specificity,accuracy,negative predictive value and positive predictive value of UE were all significantly higher than those of TI-RADS,the differences were statistically significant（P〈0.05）.AU C,sensitivity,specificity,accuracy,negative predictive value and positive predictive value of TI-RADS combined with UE were significantly higher than that of single UE or TI-RADS,the differences were statistically significant（P〈0.05）. Conclusion TI-RADS and UE have certain effect on differentiation and diagnosis of benign thyroid nodules and malignant thyroid nodules, but diagnostic value of UE was higher.And combination of TI-RADS and UE can improve accuracy in diagnosis of benign and malignant thyroid nodules.
