吉兰-巴雷综合征患者脑脊液IgG指数与蛋白水平的相关性研究

摘要 目的探讨吉兰-巴雷综合征(GBS)患者脑脊液免疫球蛋白G(IgG)指数与脑脊液蛋白水平的关系及其临床意义。方法根据脑脊液蛋白水平,将57例GBS患者分为脑脊液蛋白正常组(16例)与异常组(41例),比较2组患者的临床特征[性别、年龄、上呼吸道感染、胃肠感染、残疾量表评分(≥3分)、颅神经症状及自主神经功能症状等]及生化特征[脑脊液蛋白、脑脊液白蛋白/血清白蛋白(QALB)、IgG、IgG指数及24h鞘内IgG合成率等]的变化;根据脑脊液IgG指数将57例患者分为脑脊液IgG指数正常组(43例)与异常组(14例),比较2组脑脊液蛋白的变化;采用Peason相关分析分析脑脊液IgG指数与脑脊液蛋白的相关性。结果 GBS患者脑脊液蛋白正常组临床特征与异常组比较差异无统计学意义(P>0.05);2组脑脊液蛋白、QALB、IgG、IgG指数及24h鞘内IgG合成率等生化特征比较差异均有统计学意义(均P<0.05)。脑脊液IgG指数正常组脑脊液蛋白水平与异常组比较差异有统计学意义(U=129.00,P<0.01)。Peason相关性分析:脑脊液IgG指数与脑脊液蛋白呈正相关(r=0.377,P<0.01)。结论 GBS患者脑脊液IgG指数越大脑脊液蛋白水平越高,两者的关系有助于GBS患者病情监测及预后评估。 Objective To explore the correlation between cerebrospinal fluid immunoglobulin G (IgG) index and protein level in patients with Guillain-Barre syndrome(GBS). Methods According to the cerebrospinal fluid protein levels, 57 patients with GBS were divided into normal protein level group( 16 patients) and abnormal protein level group (41 patients). Clinical characteristics (gender, age, upper respiratory tract infection, gastrointestinal infection, disability scale score≥3, cranial nerve symptom, autonomic nervous function sytnptofia, etc. ) and biochemical indicators (eerebrospinal fluid protein, quotient of albumin (QALB), IgG, IgG index, 24 h-intrathecal IgG synthesis,etc. ) were compared between the two groups. According to the cerebrospinal fluid IgG indexes,these patients were divided into normal IgG index group(43 patients) and abnormal IgG index group(14 patients). Cerebrospinal fluid protein levels were compared between the two groups. The correlation between cerebrospinal fluid IgG index and protein level was analyzed using Peason correlation analysis. Results There were no significant differences in clinical characteristics between normal protein level group and abnormal protein level group(P〉0.05). However, the differences in biochemical indicators were significant between the two groups (all P〈 0.05). In addition, there was a significant difference in cerebrospinal fluid protein level between normal IgG index group and abnormal IgG index group(U= 129.00, P〈0.01). Peason correlation analysis showed that cerebrospinal fluid IgG index was positively correlated with cerebrospinal fluid protein level(r = 0. 377, P 〈 0.01). Conclusion The increased cerebrospinal fluid protein level is accompanied by the increased IgG index in patients with GBS. The correlation contributes to the monitoring and prognosis of GBS.
