开颅血肿清除加去骨瓣减压术在高血压脑出血手术治疗中的效果分析

Analysis of Effect of Craniotomy Hematoma Removal Combined with Decompressive Craniectomy for Hypertensive Cerebral Hemorrhage

目的探析开颅血肿清除加去骨瓣减压术在高血压脑出血手术治疗中的效果。方法整群选取该院2013年1月—2015年6月收治的55例高血压脑出血患者,将其随机分为治疗组(27例)和对照组(28例),治疗组采取开颅血肿清除术加去骨瓣减压术治疗,对照组采取单纯开颅血肿清除术治疗,对比两组患者治疗后效果。结果治疗组优8例,良11例,其优良率为70.37%,对照组优4例,良6例,其优良率为35.71%,两组比较,差异有统计学意义(P<0.05);治疗后两组患者的NIHSS评分明显较治疗前降低,且治疗组较对照组更加显著(P<0.05);治疗后,治疗组的术后ADL日常生活能力分级较对照组显著较优(P<0.05)。结论开颅血肿清除加去骨瓣减压术治疗高血压性脑出血疗效显著,可以显著改善患者神经功能缺损症状,提高日常生活能力,值得应用。

Objective To analysis the effect of craniotomy hematoma removal combined with decompressive craniectomy for hypertensive cerebral hemorrhage. Methods 55 patients with hypertensive cerebral hemorrhage admitted to our hospital from January 2013 to June 2015 were divided randomly into treatment group （n=27） and control group （n=28）. The treatment group adopted craniotomy hematoma removal combined with decompressive craniectomy, while the control group underwent simply the craniotomy hematoma removal. The effect of the two groups of patients after treatment was compared. Results Excellent result was obtained in 8 patients and good result was obtained in 11 in the treatment group with a excellent-and-good rate of 70.37%, while in the control group, 4 patients got excellent result and 6 got good result with a excellent-and-good rate of 35.71%, and the difference between the two groups was statistically significant, P〈0.05;the NIHSS score of the two groups both decreased evidently after treatment, but the treatment decreased more significantly than the control group, P〈0.05. After treatment, the ADL grade is higher in the treatment group than in the control group, P〈0.05. Conclusion Craniotomy hematoma removal combined with decompressive craniectomy for hypertensive cerebral hemorrhage is worthy of application because it can significantly improve patients＆#39; neural functional impairment and enhance their ADL.
