吉非替尼维持治疗联合化疗对晚期非小细胞肺癌患者生存期的影响

Effect of Gefitinib Maintenance Therapy Combined with Chemotherapy on the Survival Time of Patients with Advanced Non-small Cell Lung Cancer

目的研究吉非替尼维持治疗联合化疗对晚期非小细胞肺癌患者生存期的影响。方法随机抽选2012年7月—2013年7月就诊于该院的90例晚期非小细胞肺癌一线化疗后非PD患者,按随机数字表法将上述患者分为对照组(n=45)和观察组(n=45),所有患者均接受单药化疗治疗(紫杉醇),观察组加用吉非替尼维持治疗。治疗后随访记录两组患者疾病进展时间、中位生存期及生存率,并进行比较。结果观察组疾病进展时间(5.2±1.2)月、中位生存期(22.1±3.1)月与对照组(3.4±1.3)月、(15.4±3.6)月比较,差异均有统计学意义(P<0.05);观察组1年生存率62.2%、2年生存率43.3%均高于对照组35.6%、23.8%,差异有统计学意义(P<0.05)。结论吉非替尼维持治疗联合化疗能显著延长晚期非小细胞肺癌患者生存期,但该方案对复杂病例的疗效需进一步研究探索。

Objective To study the effect of gefitinib maintenance therapy combined with chemotherapy on the survival time of patients with advanced non-small cell lung cancer. Methods 90 cases of advanced non-small cell lung cancer without PD who received first-line chemotherapy and were admitted into the hospital during July 2012 to July 2013 were selected and divided into the control group （n=45） and the observation group （n=45） by the random number table method. All pa-tients were treated with monotherapy （taxol） while the observation group were additionally treated with gefitinib maintenance therapy. After the treatment, the two groups were followed up and the time of disease progression, median survival time and survival rate were recorded and compared. Results The time of disease progression in the observation group was （5.2 ±1.2） months and the median survival time was （22.1±3.1） months, compared with （3.4±1.3）months and （15.4±3.6） months in the control group, the difference was statistically significant （P〈0.05）. The 1-year survival rate of the observation group was 62.2% and the 2-year survival rate was 43.3%, which was higher than 35.6% and 23.8% of the control group, respectively, and the difference was statistically significant （P〈0.05）. Conclusion Gefitinib maintenance therapy combined with chemotherapy can significantly prolong the survival time of patients with advanced non-small cell lung cancer, but the cu-rative effect of the treatment for complex cases should be further explored.
