缺血性脑卒中和出血性脑卒中患者发病前后的血压变化研究

Changes of Patient's Blood Pressure before and after Ischemic and Hemor-rhagic Stroke

目的探讨缺血性脑卒中与出血性脑卒中患者发病前后的血压变化差异。方法整群选取2013年6月—2015年6月该院收治的脑卒中患者136例作为研究对象。其中出血性脑卒中69例,缺血性脑卒中67例,采集两组患者卒中发作后的收缩压与舒张压并比较,分析不同亚型脑卒中患者血压的差异性及其临床意义。结果出血性脑卒中患者发作后舒张压与收缩压均显著高于缺血性脑卒中,P<0.05,差异具有统计学意义;不同亚型脑卒中患者血压正常比例存在显著差异,并且两组患者各级高血压构成比存在差异,各级高血压致出血性脑卒中发病风险明显高于缺血性脑卒中,P<0.05,差异具有统计学意义。结论缺血性脑卒中与出血性脑卒中发病前后血压均存在显著差异,其中高血压对出血性脑卒中的危险度高于缺血性脑卒中,并且出血性脑卒中发病风险随血压分级的增高而增强。

Objective To study the changes of patient＇s blood pressure before and after ischemic and hemorrhagic stroke. Methods 136 patients with stroke admitted to our hospital from June 2013 to June 2015 were selected as the research ob-ject. 69 of them suffered from ischemic stroke and the other 67 suffered from hemorrhagic stroke. The systolic blood pres-sure and diastolic blood pressure in patients of the two groups after stroke were collected and compared. The difference of blood pressure between patients with different subtype of stroke and its clinical significance were analyzed. Results The systolic blood pressure and diastolic blood pressure were obviously higher in patients with hemorrhagic stroke than in those with ischemic stroke, and the difference between them was statistically significant, P〈0.05;statistically significant difference can be found on the proportion of patients with normal blood pressure between patients with different subtype of stroke, and there was statistically significant difference in proportion of patients with hypertension at all levels between the two groups;patients with hypertension at all levels showed higher incidence in occurring hemorrhagic stroke than in occurring ischemic stroke, and the difference was statistically significant, P〈0.05. Conclusion Statistically significant difference can be found on blood pressure between hemorrhagic stroke patients and ischemic stroke patients. Hypertension is more dangerous for patients with ischemic stroke than for those with hemorrhagic stroke, in addition, the risk of hemorrhagic stroke increases with the increasing of the blood pressure levels.
