摘要
目的分析妊娠期高血压的夜间血压变化规律,探讨这种夜间血压变化与夜间睡眠呼吸暂停低通气综合征的关系及其对胎儿生长发育的影响。方法比较两组孕妇的醒后睡前血压变化、夜间睡眠呼吸指标及胎儿生长发育情况,分析PIH孕妇夜间血压变化规律及其与SAHS的关系,探讨其对胎儿生长发育的影响。结果 PIH组孕妇的醒后睡前的舒张压差值大于非PIH组(P=0.01)。MSp O2每下降1%,或血压每升高5mm Hg,醒后睡前血压差值约缩小1mm Hg。PIH有晨起血压升高的趋势(P<0.05);PIH的血压节律与SAHS不相关(P>0.05);PIH的晨起血压升高不会使胎儿的IUGR风险增高(P>0.05)。结论 PIH孕妇有晨起血压的升高;PIH孕妇的血压节律改变与SAHS不相关;PIH孕妇的血压节律改变不影响胎儿的生长发育。
Objective To analyze laws of night blood pressure changes in pregnancy-induced hypertension(PIH) and to explore the correlation of night blood pressure changes and sleep apnea-hypopnea syndrome(SAHS) and its effect on fetal growth. Methods Blood pressure changes after waking up and before sleeping,night sleeping respiration index and fetal growth of pregnant in two groups were compared and correlation of night blood pressure changes of PIH pregnant women and SAHS was analyzed to explore its effect on fetal growth. Results Difference value of diastolic blood pressure after waking up and before sleeping of pregnant women in PIH group was higher than that of the non-PIH group(P=0.01).Difference value of diastolic blood pressure after waking up and before sleeping was decreased by 1mm Hg when MSp O2 was decreased by 1% or blood pressure was increased by 5mm Hg. PIH had a tendency of increasing blood pressure when people got up in the morning(P〈0.05).Blood pressure rhythm of PIH was uncorrelated to SAHS(P〉0.05). Increasing blood pressure when pregnant women got up in the morning wouldn't increase IUGR risk of fetus(P〉0.05). Conclusion PIH pregnant women have increasing blood pressure when they get up in the morning.In addition,their blood pressure rhythm change is uncorrelated to SAHS which has no effect on fetal growth.
