摘要
目的探讨超声造影在初步诊断输卵管源性不孕中的临床应用价值。方法收集2013年7月~2015年4月临床诊断考虑输卵管源性不孕症患者行超声造影267例作为研究对象。以腹腔镜通染检查为金标准,分析超声造影在初步诊断输卵管源性不孕中的价值。结果腹腔镜下通染液共检查527条输卵管,其中输卵管通畅227条,占42.97%,输卵管不通142条,占26.94%,输卵管通而不畅158条,占29.98%。Hy Co Sy检查结果通畅191条,符合率为84.1%,通而不畅111条,符合率为70.3%,不通128条,符合率为90.1%,Hy Co Sy检查结果总符合率为81.6%。与"金标准"比较,Hy Co Sy检查输卵管总符合率差异无统计学意义(x2=3.508,P〉0.05),说明Hy Co Sy检查输卵管性不孕准确率高。结论子宫输卵管超声造影能准确判断输卵管通畅性,可重复性强,无明显过敏反应,无辐射,不影响受孕时间,具有很好的临床应用价值。
Objective To explore clinical application value of contrast-enhanced ultrasound in primary diagnosis of fallopian tubal infertility. Methods 267 patients with fallopian tubal infertility who were primarily diagnosed with consideration of being received contrast-enhanced ultrasound from July 2013 to April 2015 were selected as research objects.Value of contrast-enhanced ultrasound in the primary diagnosis of fallopian tubal infertility was analyzed with laparoscopic recanalization staining examination as the gold standard. Results 527 fallopian tubes were examined by laparoscopic recanalization staining liquid.Among them,there were 227 with tubal patency accounting for 42.97%,142 with tubal impatency accounting for 26.94% and 158 with tubal partial patency accounting for 29.98%.According to Hy Co Sy examination, there were 191 with tubal patency whose coincidence rate was 84.1%,111 with tubal partial patency whose coincidence rate was 70.3%,128 with tubal impotency whose coincidence rate was 81.6%.Total coincidence rate of Hy Co Sy examination was 81.6%.Total coincidence rate of Hy Co Sy examination of fallopian tube had no statistical significance compared with gold standard(x2=3.508,P〉0.05)which showed that Hy Co Sy examination of fallopian tubal infertility had high accuracy. Conclusion Uterus and fallopian tube contrast-enhanced ultrasound can accurately determine the patency of fallopian tube and it has no allergic reaction and radiation.In addition,it does not affect the conception of time, worthy of clinical application.
