摘要
目的:针对现有取肋软骨手术创伤大、颗粒软骨制备方式效率低的实际问题,研制一种新型的可经微创切口直接切取软骨碎片的旋切器械。方法:根据现有医用旋切器及刨削刀的构造和工作原理,首先在Auto CAD环境下构建旋切刀头,后以金属3D打印成形,再根据设计的结构组装成电动肋软骨旋切器。将该器械分别在尸体和猪活体进行实验性操作,以观察旋切效果。将用该器械从猪体内取出的软骨碎片和传统方法制备的颗粒软骨分别以Ⅱ型胶原酶消化分离软骨细胞,进行台盼蓝染色计数。结果:成功组装出国内首台电动肋软骨旋切器。可在尸体及活体猪胸廓以旋切的方式直接获取肋软骨碎片,对周围组织损伤较小。旋切所得软骨碎片和由传统刀片制备的颗粒软骨经消化分离后的软骨细胞活率无统计学差异(P>0.05)。结论:电动肋软骨旋切器可经小切口旋转切取肋软骨碎片,经初步验证,该旋切器具有有效性和安全性。
Objective To develop a new type of cartilage rotary cutter,which can be used to directly obtain cartilage fragments via small incision,aiming at the current problems of large injury with rib cartilage obtaining procedure and low efficiency with diced cartilage preparation.Methods According to the structures and working principles of the existing medical rotary cutters,a digital model of rotary cutter head was built in Auto CAD program.Then a metal head was made by 3D printing technology according to the digital model.Afterward,with combinations with cutter head,cannula and power system,an electric cartilage rotatory cutter device was assembled.Experimental operating in cadaver and pig was assigned to test the possibility of the device.Cartilage fragment product of this rotary cutter device was sent for chondrocyte isolation with Collagenase Ⅱ and Dead/Live cell counting with trypan blue staining.Results An electric cartilage rotary cutter device was invented,which could directly cut off cartilage pieces from costal cartilage in both cadaver and pig,while causing minimal injury on peripheral tissue.As to the viability of chondrocytes,there was no significant difference between the cartilage pieces cut by rotary cutter and those by surgical blade(P0.05).Conclusion This electric cartilage rotary cutter device can be used to directly harvest cartilage pieces via small incision with minimal injury on perheral tissue,which approved the effectiveness and safety of this device preliminarily.
