骨水泥型和生物型股骨假体半髋置换治疗高龄移位型股骨颈骨折的疗效观察

Efficacy Observation of Hemiarthroplasty with Cemented and Biological Femoral Prosthesis in Treatment of Elderly Displaced Femoral Neck Fractures

目的比较骨水泥型和生物型股骨假体半髋关节置换治疗高龄移位型股骨颈骨折的临床疗效。方法选择我院2012年6月至2014年6月收治的高龄移位型股骨颈骨折患者64例,根据患者应用的不同置换材料将其分为A组(通过骨水泥型股骨假体进行置换)和B组(通过生物型股骨假体进行置换),每组各32例。比较两组的手术相关情况和并发症发生情况。结果两组患者的术中出血量、术后引流量比较均无显著差异(P>0.05),但A组患者的手术时间显著长于B组,差异有统计学意义(P<0.05)。A组并发症发生率为28.13%,B组为31.25%,两组比较差异不显著(P>0.05)。两组患者术后6个月、12个月的Harris评分比较无显著性差异(P>0.05)。结论临床上应根据患者自身的身体状况去选择最适合的股骨假体来实施置换手术。

Objective To compare the clinical effects of hemiarthroplasty with cemented and biological femoral prosthesis in treatment of elderly displaced femoral neck fractures. Methods 64 elderly patients with displaced femoral neck fractures treated in our hospital from June 2012 to June 2014 were randomly divided into group A（given cemented femoral prosthesis） and group B（given biological femoral prosthesis） according to different replacement materials, with 32 cases in each group. The operation related indices,complications of two groups were compared. Results The intranperative bleeding volume and postoperative drainage volume of two groups had no statistical（P〈0.05）, but the operation time of group A was higher than group B, with statistical difference（P〈0.05）. The incidence of complications of group A and group B were 28.13%, 31.25% respectively, the difference was not statistical（P〈0.05）. The Harris scores of two groups 6 months and 12 months after treatment had no statistical difference（P〉0.05）. Conclusions The selection of most suitable femoral prosthesis for replacement surgery should be based on patient＇s own physical conditions in clinic.
