氯诺昔康联合地佐辛肌注用于犬创伤后镇痛疗效观察 被引量:2

摘要 目的:观察肌注氯诺昔康、地佐辛及两者联合用药对犬创伤后院前镇痛的影响。方法中华田园犬16只,雌雄不限,体质量7~10 kg,按三因素4×4拉丁方设计随机分成4组:A 组:肌注氯诺昔康0.4 mg/ kg;B 组:肌注地佐辛0.5 mg/ kg;C 组:肌注氯诺昔康0.2 mg/ kg +地佐辛0.25 mg/ kg;D 组:肌注氯诺昔康0.4 mg/ kg +地佐辛0.5 mg/ kg,每组4只。各组模拟脾切除术建立模型,之后即刻单次肌注药物,分别于术前1 h(T0)和给药后2 h(T1)、4 h(T2)、6 h(T3)、8 h(T4)和24 h(T5)测定疼痛评分、心率(HR)和呼吸频率(RR),并检测血气、血糖(GLU)、血清皮质醇(S)、肾上腺素(E)、白细胞介素-10(IL-10)和α肿瘤坏死因子(TNF-α)。结果(1)疼痛评分:T1时点,B 组墨尔本大学疼痛总评分(MPS)和客观疼痛评分高于其余3组(P 〈0.05)。T2时点,A 组客观疼痛评分中动作项目评分高于其余3组(P 〈0.05)。T2和 T3时点,B 组 MPS 高于 C、D 两组(P 〈0.05)。T4时点,A 组客观疼痛评分中动作项目评分高于 C、D 两组(P 〈0.05)。T5时点,B 组 MPS 中吠叫项目评分高于其余3组(P 〈0.05)。(2)其他指标:T4时点,B 组血糖值高于 C、D 两组(P 〈0.05)。T5时点,A 组血糖值高于 C、D两组(P 〈0.05);B 组 TNF-α值高于其 T0时的起始值(P 〈0.05)。结论4组镇痛方案中,D 组评价最好。氯诺昔康和地佐辛联合用药具有相加的镇痛效果及较好的协同效应。 Objective To observe the ana1gesic effects of intramuscu1ar 1ornoxicam,dizocine,and their combinations on dogs of post-trauma at ear1y stage. Methods 16 Adu1t Chinese rura1 dogs of both sexes,weighing 7-10 kg,were random1y divided into 4 groups (according to three factors of 4 × 4 Latin square design):group A(0. 4 mg/ kg 1ornoxicam),group B(0. 5 mg/ kg dizocine),group C (0. 2 mg/ kg 1ornoxicam combinated with 0. 25 mg/ kg dizocine),group D(0. 4 mg/ kg 1ornoxicam combinated with 0. 5 mg/ kg dizo-cine). Post-trauma pain mode1 was produced by ana1ogic sp1enectomy and then the durg of each group was administered immediate1y by sing1e intramuscu1ar injection. Pain scores,heart rate and respiratory rate,b1ood gas ana1ysis,and b1ood samp1es such as g1ucose,se-rum cortiso1,epinephrine,TNF-α and IL-10 were monitored at T0(1 h before operation),T1(2 h after injection),T2(4 h after injec-tion),T3(6 h after injection),T4(8 h after injection)and T5(24 h after injection)respective1y. Results (1)Pain scores:at T1 ,Me1-bourne pain score(MPS)and objective pain score in group B were higher than those of the other three groups(P 〈 0. 05). At T2 ,the action of objective pain score in group A was higher than that of the other three groups(P 〈 0. 05). At T2 and T3 ,MPS in group B was higher than that of group C and group D(P 〈 0. 05). At T4 ,the action of objective pain score in group A was higher than that of the group C and group D(P 〈 0. 05). At T5 ,bark of MPS in group B was higher than that of the other three groups(P 〈 0. 05).(2)Oth-ers:at T4 ,G1u in group B was higher than that of group C and group D(P 〈 0. 05). At T5 ,G1u in group A was higher than that of group C and group D(P 〈 0. 05),and TNF-α of T5 was higher than that of T0 in group B(P 〈 0. 05). Conclusion Group D is the best in this study. Lornoxicam comined with dizocine may have favourab1e additive or synergistic ana1gesic effect in our study.
