鼻内镜下带蒂鼻中隔黏骨膜瓣转位联合游离下鼻甲黏膜瓣修补鼻中隔穿孔

The repair of nasal septum perforation by combining pedicled nasoseptal mucoperiosteal flap transposition with free inferior turbinate mucosal flap under nasal endoscope

目的探讨鼻内镜下行带蒂鼻中隔黏骨膜瓣转位联合游离下鼻甲黏膜瓣修补鼻中隔穿孔的疗效及分析手术经验。方法 2006年2月-2013年12月我院收治的鼻中隔穿孔患者12例,在鼻内镜下鼻中隔穿孔一侧行带蒂鼻中隔黏骨膜转位贴补联合另一侧游离下鼻甲黏膜组织瓣贴补术,术后随访观察鼻中隔穿孔修补创面愈合情况及修补效果。结果 12例鼻中隔穿孔均一期修补成功,随访3个月以上未见再穿孔。结论鼻内窥镜下行带蒂鼻中隔黏骨膜瓣转位联合游离下鼻甲黏膜瓣修补鼻中隔是一种操作方便、安全、可靠的方法。

Objective To explore the clinical efficacy of using the rotated pedicle flap of the periosteum of nasal septum combined with the free mucosa of inferior turbinate under the nasal endoscope to repair the perforation of nasal septum. Methods Endoscopic operation was performed in 12 patients suffering from nasal septum perforation from Feb 2006 to Dec 2013. Perforation of nasal septum was covered with the rotated pediele flap of the periosteum of nasal septum in one side of the perforation. In the other side of the perforation, the free mueosa of inferior turbinate prepared was sutured to the periosteum of nasal septum. Cure of the wound and clinical efficacy of the operation was obse,~ ed during the follow-up. Results The perforation of nasal septum in all 12 patients was closed successfully after the surgery. Each patient was followed up for more than 3 months, during which no reperforation occurred. Conclusion Endoscopic operation of repairing nasal septum perforation with the rotated pedicle flap of the periosteum of nasal septum combined with the free mucosa of inferior turbinate is proved to be a simple, safe and reliable method.
