红细胞分布宽度和B型脑钠肽对评价2型糖尿病患者心功能的作用 被引量:1

Effect of red cell distribution width and serum BNP on the evaluation of cardiac function in patients with diabetes mellitus

摘要 目的探讨2型糖尿病合并心力衰竭的患者血红细胞分布宽度(RDW)与B型脑钠肽(BNP)对心功能的评价作用。方法以2013年1至12月在第二军医大学附属长海医院住院治疗的2型糖尿病合并心力衰竭患者为研究组(134例),不合并心力衰竭的2型糖尿病患者为对照组(126例),检测其RDW、BNP及血红蛋白水平,并以超声心动图评价其心功能。比较两组间实验室检查指标的差异,并分析RDW与BNP的相关性。结果研究组患者RDW和BNP水平均显著高于对照组(P<0.01),而两组间血红蛋白水平差异无统计学意义(P>0.05)。研究组RDW与BNP水平呈显著正相关(P<0.01),而与血红蛋白水平不相关(P>0.05)。对照组RDW与BNP及血红蛋白水平均无相关性(P>0.05)。结论 2型糖尿病合并心力衰竭患者RDW与BNP水平显著相关,RDW是2型糖尿病患者识别和评价心力衰竭的指标之一。 Objective To explore the effect of red cell distribution width ( RDW) and B-type natriuretic peptide ( BNP) on evalua-tion of cardiac function in type 2 diabetes mellitus ( DM) patients accompanied with heart failure. Methods Patients with type 2 DM admitted in Changhai Hospital from January 2013 to December 2013 with clinical and echocardiographic features of heart failure were included as case group. DM patients without heart failure were included as control group. RDW,serum BNP and hemoglobin level were detected. Echocardiography was used to evaluate cardiac function. Differences of laboratory index between the two groups and the corre-lation between RDW and BNP in each group were analyzed. Results There were 134 cases in research group,and 126 cases in con-trol group. RDW and BNP levels were significantly higher in case group compared with control group (P〈0. 01),and no significant difference of hemoglobin was shown between the two groups (P=0. 092). RDW positively correlated with BNP level significantly in case group (P〈0. 01),and not correlated with hemoglobin level (P〉0. 05). In control group,RDW showed no significant correlation with both BNP and hemoglobin (P〉0. 05). Conclusion There was a significant correlation of RDW and BNP in type 2 DM patients with heart failure,and RDW could be used as a recognition and evaluation indicator of heart failure in such patients.
