探讨慢性肺心病患者社区治疗及管理

目的分析探讨对慢性肺心病患者实施社区治疗与管理的干预效果。方法针对社区慢性肺心病患者进行疾病相关管理,分析实施管理前后患者的疾病知识以及生活质量的改善情况。结果实施健康档案管理、疾病治疗管理、随访调查等管理方案后,患者对疾病知识的知晓率以及生活质量得到有效改善,健康意识逐渐增强。结论对慢性肺心病患者实施社区治疗及管理,能够实现慢性病患者管理的规范化,有利于实现"医院-社区-患者"的管理模式、提高治疗及管理的质量以及提高患者的健康意识。

Objective To investigate the effect of the intervention in patients with chronic implement community-based treatment and management. Methods In order to carry out disease related management in patients with chronic pulmonary heart disease in community,and analyze the knowledge of disease and the improvement of life quality before and after the implementation of the management of patients with chronic pulmonary heart disease. Results After the implementation of health records management,disease treatment management,follow-up survey and other management programs,the patient＇s awareness of disease knowledge and the quality of life has been effectively improved,health awareness gradually increased. Conclusion In the treatment of chronic pulmonary heart disease patients,the implementation of community treatment and management,can achieve the standardization of chronic diseases,is conducive to the realization of the “Hospital-Communi-ty-patients”management mode,improve the quality of treatment and management and improve the health of patients.
