不同剂量的米非司酮治疗异位妊娠临床对比研究

Clinical Comparative Research on Different Doses of Mifepristone in Treatment of Heterotopic Pregnancy

目的对比及分析不同剂量的米非司酮治疗异位妊娠的临床疗效。方法整群选取2013年11月—2014年11月在该院接受并确诊的异位妊娠患者120例,随机的分为A、B、C 3组,各40例,3组患者分别口服100、200、400 mg的米非司酮,治疗3个疗程后进行对比分析2组临床治疗效果,并统计2组患者异位妊娠血与β-HCG的关系。结果治疗后3组患者的临床症状均明显改善缓解,其中A组患者的总有效率为50.0%,B组的总有效率为80.0%,C组的总有效率为95.0%,3组比较差异有统计学意义（P〈0.05）;在患者血β-HCG与米非司酮的疗效关系中,当血β-HCG浓度为2 001~5 000 U/L时,疗效最高为100%,剂量为400 mg;当血β-HCG浓度为300~2 000 U/L时,疗效最高为68.8%,剂量为200mg,A组、B组、C组两两比较差异具有统计学意义（P〈0.05）。结论米非司酮治疗异位妊娠疗效确切,且血β-HCG浓度越高时用剂量越大的米非司酮时,疗效越显著,值得在临床中推广使用。

Objective To compare and analyze the clinical curative effect of different doses of mifepristone in treatment of heterotopic pregnancy. Methods 120 cases of patients with hcterotopic pregnancy confirmed in our hospital from November 2013 to November 2014 were selected and divided into group A, group B and group C with 40 cases in each, the three groups were respectively given oral 100mg, 200mg and 400mg mifepristone, the clinical treatment effect of the three groups were compared and analyzed after 3 courses of treatment, and the relationship between heterotopic pregnancy blood and β-HCG of the three groups was counted. Results The clinical symptoms of the three groups after treatment were obviously improved, the total effective rate was 50.0% in group A, 80.0% in group B and 95.0% in group C, the differences were obvious by comparison （P〈0.05）, when the concentration of serum β-HCG was 2001-5000U/L, the highest curative effect was 100%, and the dosage was 400mg; when the concentration of serum β-HCG was 300-2000U/L, the highest curative effect was 68.8%, and the dosage was 200mg; the pairwise comparison of group A, group B and group C was statistically significant （P〈0.05）. Conduslon Mifepristone in treatment of heterotopic pregnancy has a definite curative effect, as the concentration of serum β-HCG becomes higher ,the higher doses of mifepristone can make the curative effect more obvious, which is worthy of promotion and application in clinic.
