利用斑点追踪技术对原发性扩张型心肌病患者右心房功能的研究

Research on Speckle Tracking Imaging for Right Atrial Function in Patients with Primary Dilated Cardiomyopathy

目的探究斑点追踪在对扩张型心肌病患者右心房功能改变中的临床应用价值。方法整群选择2013年7月—2014年6月在该院诊断为扩张型心肌病的患者作为研究对象。扩张型心肌病患者为观察组24例。正常人为对照组24例。对观察对象的心脏相关数据进行监测。结果扩张型心肌病患者的左室射血分数为(34.2±8.0),E’/A’比值、右心室面积变化分数、右室短轴缩短率,较对照组低,E/E’比值、右心室心肌高于对照组,右房整体纵向应变低于对照组。P<0.05为差异有统计学意义。右房整体纵向应变与三尖瓣环收缩期位移呈正相关,与右心室心肌做功指数呈负相关。结论右房整体纵向应变与右心的收缩功能具有很强的关联性,可以通过二维斑点追踪技术对右房的应变能力进行及早评估,发现扩张型心肌病的右心功能受损情况。

Objective To study the clinical application value of speckle tracking imaging for the changes of right atrial function in patients with primary dilated cardiomyopathy. Methods 48 cases of patients with dilated cardiomyopathy diagnosed in our hospital from July 2013 to June 2014 were selected as the research object and divided into two groups with 24 cases in each, the patients with dilated cardiomyopathy were regarded as the observation group, the normal people were regarded as the control group, the data related cardiac of patients were monitored. Results The left ventricular ejection fraction was （34.2±8.0） in patients with dilated cardiomyopathy, the E/A ratio, right ventricular area change fraction, shortening rate of right ventricular short axis in the observation group were lower than those in the control group, E/E ＂ratio and myocardium of right ventricle in the observation group were higher than those in the control group, overall longitudinal strain of right atrium in the observation group was lower than that in the control group, P〈0.05, all differences were statistically significant, , overall longitudinal strain of right atrium was positive correlated with tricuspid annulus systolic displacement, and was negative correlated with myocardium of right ventricle work index. Conclusion The overall longitudinal strain of right atrium was strongly correlated with right ventricular systolic function, the strain capacity of right atrium can be early evaluated and the right cardiac function damage of dilated cardiomyopathy can he found by two-dimensional speckle tracking imaging.
