根治术与保乳术后放疗用于早期乳腺癌患者的疗效对比

Comparison of curative effect of postoperative radiotherapy for early breast cancer patients

目的探究根治术与保乳术联合术后放疗治疗早期乳腺癌的疗效对比。方法研究对象选取2008年1月~2011年1月之间我院收治的112例Ⅰ期、Ⅱ期乳腺癌患者,根据手术方式不同分为根治术组71例和保乳术组41例,根治术组患者采用改良根治术治疗,保乳术组患者实施保乳手术并术后给予全乳适形调强放疗,两组术后均采用CEF方案进行化疗,对比两组患者的3年生存率、远端转移、局部复发情况。结果根治术组患者的3年生存率为94.4%（67/71）,远端转移率为4.2%（3/71）,局部复发率为7.0%（5/71）;保乳术组患者的3年生存率为90.2%（37/41）,远端转移率为9.8%（4/41）,局部复发率为7.3%（3/41）,两组患者的3年生存率和局部复发率对比无显著差异,但保乳术组患者的远端转移率显著高于根治术组患者（P〈0.05）。结论保乳手术联合术后放疗相较于根治术而言,对乳腺癌患者术后的影响较小,且3年生存率、局部复发情况与根治术相近,但远端转移率较高,可根据情况进行选择。

Objectives To explore the effect of radical mastectomy combined with breast conserving surgery in the treatment of early breast cancer. Methods 112 patients with Ⅰ and Ⅱ were selected from 2008. 1 -2011.1,41 cases of breast cancer, 71 ca- ses of breast conserving surgery, eases of breast conserving surgery and postoperative radiotherapy. The two groups were treated with CEF. The 3 year survival rate, distant metastasis and local recurrence were compared between the two groups. Results The 3 year survival rate was 94.4% （67/71）, 4. 2% （3/71）, 7% years survival rate was 9.8% （37/41）, 7. 3% （4/41）, 3 years survival rate and local recurrence rate in two groups were significantly higher than those in radical operation group （ P 〈 0. 05 ）. Conclusions The effect of breast conserving surgery＇combined with postoperative radiotherapy is small, and the 3 year survival rate and local recurrence rate are similar, but the distal metastasis rate is higher, and can be chosen according to the situation.
