腹腔镜与小切口胆囊切除术治疗胆结石的效果分析

Effect analysis of laparoscopic treatment in gallstones with small incision cholecystectomy

目的探讨腹腔镜与小切口胆囊切除术治疗胆结石的临床效果。方法将本院2013年10月~2015年6月收治的62例胆结石患者,按照随机数字表法分为观察组和对照组,每组各31例,对照组患者给予小切口胆囊切除术治疗,观察组患者给予腹腔镜胆囊切除术治疗,观察并比较两组患者的手术时间、术中出血量、下床活动时间、肛门排气时间、住院时间以及并发症的情况。结果观察组患者的手术时间、下床活动时间、肛门排气时间和住院时间均显著短于对照组,术中出血量少于对照组,差异有统计学意义（P〈0.05）。两组患者均未出现胆管损伤和胆漏的病例;观察组患者感染的发生率为3.2%,低于对照组的12.9%,差异有统计学意义（P〈0.05）。结论腹腔镜胆囊切除术治疗胆结石较小切口胆囊切除术的手术时间短、术中出血量少、术后恢复快、并发症少,值得临床推广应用。

Objective To explore the clinical effect of laparoscopic treatment in gallstones with small incision cholecystectomy. Methods 62 patients with gallstones in our hospital from October 2013 to June 2015 were divided into observation group and control group according to random number table method, each group of 31 cases, the control group patients were give small incision cholecystectomy treatment, the observation group patients were treated with laparoscopic cholecystectomy, the two groups of patients with operation time, intraoperative blood loss, ambulation time, anal exhaust time, length of hospital stay, and complications were observed and compared. Results the~ observation group of patients with operation time, ambulation time, anus exhaust time and hospital stay were significantly shorter than the control group, intraoperative blood losswas less than the control group, the difference was statistically significant （P 〈 0.05）.Two groups of patients were not present cases of bile duct injury and bile leakage; The incidence of infection was 3.2% in observation group of patients, it was lower than the control group （12.9%）, the difference was statistically significant （P 〈 0.05）. Conclusion The laparoscopic cholecysteetomy in the treatment of gallstones small incision cholecystectomy has shorter operation time, less intraoperative blood loss, postoperative rapid recovery, fewer complications, it is worthy of clinical popularization and application.
