摘要
目的:以护理循证为基础建立青光眼患者延续护理模式。方法:检索2004至2014年MEDLINE数据库及中国期刊全文数据库近10年文献,归纳整理,参考国内外现有延续护理个案管理模式,征求专家意见和患者需求,制定护理方案。结果:明确青光眼出院患者应用奥马哈系统实施延续护理的内容及流程,确定青光眼患者延续护理评价指标。结论:护理模式的建立应以循证为基础,以临床需求为导向,以提高护理质量、提高患者满意度为目的。
Objective： To establish nursing model based on the evidence for glaucoma patients. Methods： We searched and categorized MEDLINE 2004-2014 database and Chinese journal full text database for nearly 10 years, referenced domestic and international transitional care case management model, seek for advice from experts and considered needs of patients, developed nursing program. Results： The content and process of transitional nursing care for the patients with glaucoma were defined, and the evaluation index was determined also. Conclusion： The nursing model base on evidence and adapt to clinical demand, can help to improve patient satisfaction and nursing quality.
