窒息新生儿检测血清胰岛素样生长因子-1及其结合蛋白-3和Ghrelin的意义 被引量:2

摘要 目的研究窒息新生儿检测血清胰岛素样生长因子-1(IGF-1)及其结合蛋白-3(IGFBP-3)和Ghrelin的意义。方法选择窒息新生儿48例作为窒息组,选择同期出生的正常新生儿33例作为正常组。检测窒息组治疗前后和正常组血清IGF-1、IGFBP-3、Ghrelin和胰岛素水平,分析血清IGF-1、IGFBP-3、Ghrelin和胰岛素水平与窒息程度关系和各指标之间的关系。结果窒息组治疗前IGF-1、IGFBP-3和胰岛素水平均明显低于正常组(P均<0.05),Ghrelin水平明显高于正常组(P<0.05),且随着窒息严重程度的增高,血清IGF-1、IGFBP-3和胰岛素水平明显降低(P<0.05),Ghrelin水平明显升高(P<0.05);治疗后各指标水平较治疗前明显改善(P均<0.05)。对窒息患儿血清IGF-1、IGFBP-3、Ghrelin和胰岛素水平的相关性分析发现,Ghrelin水平与IGF-1(r=-0.568,P<0.05)、IGFBP-3(r=-0.467,P<0.05)和胰岛素(r=-0.675,P<0.05)水平呈负相关,胰岛素水平与IGF-1(r=0.564,P<0.05)和IGFBP-3(r=0.649,P<0.05)水平呈正相关。结论 IGF-1、IGFBP-3和Ghrelin参与了新生儿窒息的发生发展过程,联合检测对评价新生儿窒息缺氧损伤程度具有一定的价值。 Objective It is to observe the significance of serum insulin-like growth factor- 1( IGF- 1),insulin-like growth factor binding protein- 3( IGFBP- 3) and Ghrelin in neonates patients with asphyxia. Methods 48 neonates patients with asphyxia were selected as neonatal asphyxia group. At the same period,33 newborns born were selected as neonatal group. The levels of IGF- 1,IGFBP- 3,Ghrelin and insulin were determined before and after treatment in neonatal asphyxia group and neonatal group,and their relationship with the degree of asphyxia and among the indexes were also analyzed. Results The levels of IGF- 1,IGFBP- 3 and insulin in neonatal asphyxia group were significantly lower than those in neonatal group( P〈 0. 05),and the levels of those appeared significantly lower in neonatal asphyxia group as the severity of neonatal asphyxia increased( P〈 0. 05),and were significantly higher after treatment than those before treatment( P〈 0. 05). And Ghrelin levels in neonatal asphyxia group were significantly higher than those in neonatal group( P〈 0. 05),and with the increasing severity of asphyxia the Ghrelin levels increased,and the levels of that decreased significantly after treatment than those before treatment( P〈 0. 05). Correlation analysis of serum IGF- 1,IGFBP- 3,Ghrelin and insulin in neonatal asphyxia group showed that Ghrelin levels were negatively correlated to IGF- 1( r =- 0. 568,P〈 0. 05),IGFBP- 3( r =- 0. 467,P〈 0. 05) and insulin( r =- 0. 675,P〈 0. 05),insulin levels were positively correlated to IGF- 1( r = 0. 564,P〈 0. 05)and IGFBP- 1( r = 0. 649,P〈 0. 05). Conclusion IGF- 1,IGFBP- 3 and Ghrelin involved in the development process of neonatal asphyxia,combined detection of IGF- 1,IGFBP- 3 and Ghrelin has a certain value to evaluate the extent of damage in neonatal asphyxia.
