摘要
目的:研究左氧氟沙星注射液对老年住院病人血糖的影响,为临床用药提供参考。方法:采用回顾性分析的方法,分析上海市第一康复医院2013年10月至2014年12月使用左氧氟沙星注射液治疗感染性疾病的老年住院病人的血糖异常情况。结果:入选病例634例,用左氧氟沙星后出现血糖异常者46例(7.26%),泌尿系统感染病人发生率(27/212,12.74%)显著高于其他系统感染者(P<0.01)。634例病人中合并糖尿病者72例,其中34例(47.22%)用药后出现血糖异常;而没有糖尿病的562例病人用药后仅有12人(2.14%)出现血糖异常,差别有显著意义(P<0.01)。用左氧氟沙星治疗后出现血糖异常的34例糖尿病病人中,23人血糖降低(23/34,67.65%),显著多于血糖升高和兼有降低和升高者。用胰岛素+口服降糖药、磺酰脲类药物治疗的糖尿病病人,用左氧氟沙星治疗后发生血糖异常的几率较其他药物治疗时高。结论:左氧氟沙星可显著影响用抗糖尿病药物治疗的老年糖尿病病人的血糖,主要表现为血糖降低。
Objective:To study the effect of levofloxacin injection on blood glucose level(BGL)in senile inpatients,so as to provide reference for clinical medication.Methods:A retrospective analysis method was used to analyze the BGL of the senile inpatients with infectious diseases treated with levofloxacin injection from Oct 2013 to Dec 2014 in the First Rehibitation Hospital of Shanghai.Results:Of the 634 selected inpatients,46 displayed abnormal BGL after medication of levofloxacin injection(7.26%).The incidence of urinary tract infection in the inpatients was 12.74%(27/212),which was significantly higher than that of the inpatients with other system infection(P〈0.01).In the 634 inpatients,there were 72 inpatients complicated with diabetes mellitus,of which 34inpatients(47.22%)displayed abnormal BGL after medication,while in the 562 inpatients without diabetes mellitus,only 12inpatients(2.14%)displayed abnormal BGL after medication,with statistical significance(P〈0.01).In the 34 cases that displayed abnormal BGL after medication,the BGL of 23 cases dropped(23/34,67.65%),which was significantly more than that of the inpatients with higher BGL and those coupled with increased or decreased BGL.After intravenous administration of levofloxacin,the incidence of abnormal BGL in diabetic patients treated with insulin+oral hypoglycemic agents or sulfonylurea was higher than that treated with other hypoglycemic agents.Conclusion:Intravenous administration of levofloxacin could significantly affect the BGL of the senile diabetic inpatients treated with hypoglycemic agents,with the decrease of BGL as its main manifestation.
