功能性与非功能性胰腺神经内分泌肿瘤的MSCT动态增强特征 被引量:4

摘要 目的探讨功能性与非功能性胰腺神经内分泌肿瘤(PanNETs)的MSCT动态增强特征。方法回顾分析经手术病理证实的11例功能性PanNETs及10例非功能性PanNETs的MSCT表现。计算肿瘤实性部分的动脉期、门静脉期、延迟期相对强化程度(L_A、L_P、L_D)、瘤体实性区域CT值与胰腺的比值(R_A,R_P,R_D),并进行统计学分析。结果两类瘤体最大径、胰管扩张差异均有统计学意义(P均<0.05),而发病部位、边界、囊变、钙化、脾静脉侵犯、淋巴结转移及远处脏器转移差异均无统计学意义(P均>0.05)。功能性PanNETs肿瘤实性部分的L_A、L_P、L_D差异有统计学意义(P<0.05),R_A,R_P,R_D差异亦有统计学意义(P<0.05)。非功能性PanNETs肿瘤实性部分的L_A、L_P、L_D差异无统计学意义(P>0.05),R_A,R_P,R_D差异亦无统计学意义(P<0.05)。结论功能性PanNETs一般较小,较少合并胰管扩张,动脉期和门静脉期强化程度较延迟期明显,而非功能性PanNETs三期强化程度无明显差别。 Objective To explore MSCT dynamic contrast enhancement features of functional and non-functional pancreatic neuroendocrine tumors(PanNETs).Methods MSCT manifestations of 11 functional PanNETs patients and 10 nonfunctional PanNETs patients confirmed by pathology were retrospectively analyzed.The relative enhancement level(LA、LP、LD)and the ratio of tumor and pancreas CT value(RA,RP,RD)in three phases(arterial,portal and delay phase)were calculated,and statistical analysis was performed.Results Tumor maximum diameter and dilated pancreatic duct incidence both had statistically significant differences between functional PanNETs and the non-functional PanNETs(both P〈0.05),but pathogenic site,margin,cystic change,calcification,splenic vein invasion,lymph nodes and distant metastasis all had no statistically significant differences(all P〉0.05).There were statistically significant differences among LA,LPand LDin functional PanNETs solid portion(P〈0.05),and there also were statistically significant differences among RA,RP,RD(P〈0.05).There were no statistically significant differences among LA,LP and LD in non-functional PanNETs solid portion(P〈0.05),and there also were no statistically significant differences among RA,RP,RD(P〈0.05).Conclusion The functional PanNETs likely have smaller size,less dilated pancreatic duct incidence,and more obvious enhancement in arterial and portal phase than delay phase.But no differences are found for the non-functional PanNETs in the three phases.
