摘要
目的探讨女性乳腺疼痛症患者甲状腺激素水平变化的临床意义。方法随机选取门诊131例女性乳腺疼痛症患者(病例组),同时段随机选取50例门诊健康体检的女性(对照组);均依据年龄分为<45岁与≥45岁2个层次,门诊行甲状腺及乳腺超声检查,留取是否伴发甲状腺疾患、是否腺体紊乱等资料。清晨空腹抽血,采用免疫发光法检测T3(三碘甲状原氨酸)、T4(四碘甲状原氨酸)、TSH(促甲状腺激素)、TPO(甲状腺过氧化物酶)、TG(甲状腺球蛋白)、Anti-TSHR(抗促甲状腺激素受体抗体)。比较病例组患者是否乳腺疼痛、是否伴发甲状腺疾患、是否腺体紊乱及其与对照组有无差异。结果乳腺疼痛与年龄有关,多为<45岁女性(P<0.05)。≥45岁乳腺疼痛患者合并甲状腺疾患者血清T3水平较无甲状腺疾患者高,与对照组比较差异有统计学意义(P<0.05);在≥45岁和<45岁2个分层有腺体紊乱者血清T4水平都较无腺体紊乱者高(P<0.05);T3、T4、TSH在其余各分组及分层之间比较差异均无统计学意义(P≥0.05)。病例组TPO、TG水平高于对照组(P<0.05)。病例组患者中无论是否伴发甲状腺疾患、是否伴发腺体紊乱者TPO、TG水平均高于对照组(P<0.05)。结论女性乳腺疼痛症与TPO、TG、T4水平和年龄有关。
Objective To study the clinical significance on the level of thyroid gland related hormone in hyperplasia of mammary gland(HMG)patients with breast pain.Methods 131 cases of hyperplasia of HMG patients were randomly selected as the case group from out inpatients.At the same time,while 67 cases of physical examination women were chosen as the control group randomly,all of which are stratified as 2parts by ages,ie,〈45year-old and ≥45year-old.B ultrasonic examination on thyroid gland and breast were performed and fasting blooded in the morning.serum triiodothyronine(T3),thyroxine(T4),TSH(thyroid stimulating hormone),TPO(thyroid peroxidase),TG(thyroid globulin),Anti-TSHR(anti thyroid stimulating hormone receptor antibody)were detected by immunoluminometric assay.Results The breast pain were related to the age of the patients,most of them were〈45year-old female(P〈0.05).In the case group,the concentration of serum T3at≥45year-old with thyroid disease were higher than those without thyroid disease.which the difference was statistically significant(P〈0.05).In the case group,the concentration of T4 in glandular disorder of all patients were higher than those without glandular disorder(P〈0.05).There were no statistical significance of serum T3,T4,and TSH compared in other groups and parts(P〈0.05).And the levels of TPO and TG in the case group were higher than in control group(P〈0.05).Compared between age parts,the levels of TPO and TG in case group were higher than in control group(P〈0.05).Conclusion The HMG with breast pain in female was positive correlated with the levels of TPO,TG and T4.
