超声评价慢性乙肝肝硬化与胆囊壁厚度及前后径改变的关系

Ultrasound Evaluation of Relationships between Chronic Liver Cirrhosis and Changes of Gallbladder Wall Thickness and Anteroposterior Diameter

目的:探讨超声评价慢性乙肝肝硬化与胆囊壁厚度及前后径改变的关系的效果。方法:统计分析2012年8月至2015年2月我院超声科收治的43例慢性乙肝肝硬化患者的临床资料作为研究组,选取同期进行健康体检的43例资料作为对照组,超声检查两组空腹、非脂肪餐后1h、2h胆囊壁增厚量及前后径缩短量,和肝功能Child-pugh改良分级进行比较分析。结果:研究组患者的空腹胆囊壁厚度均显著大于对照组(P<0.05);研究组患者空腹胆囊壁增厚量、胆囊前后径缩短量和肝功能Child-Pugh改良分级具有最强的相关性(P<0.05),凝血酶原时间、门静脉主干内径、腹水、脾大及肝功能中胆红素、白蛋白和肝功能分级均具有较强的相关性(P<0.05),血常规血小板计数、白细胞、红细胞、血红细胞含量随着肝功能分级的增强而提升(P<0.05),肾功能BUN、Cr、UA含量随着肝功能分级的增强而降低(P<0.05)。结论:超声能够有效评价慢性乙肝肝硬化肝功能分级与胆囊壁厚度及前后径改变的关系。

Objective： To investigate the effect of ultrasound evaluation of relationships between chronic liver cirrhosis and changes of gallbladder wall thickness and anteroposterior diameter. Method： The clinical data of 43 cases of patients with chronic liver cirrhosis treated in ultrasound department of our hospital from August 2012 to February 2015 were statistically analyzed. Result： The gallbladder wall thickness of the study group was significantly higher than that of the control group（ P〈0. 05）; The fasting gallbladder wall thickening amount,anteroposterior diameter shortened and improved liver function Child- Pugh classification had the strongest correlations（ P〈0. 05）; portal vein diameter,splenomegaly,ascites,prothrombin time and liver function bilirubin,albumin,and liver function had stronger correlations（ P〈0. 05）; blood leukocytes,erythrocytes,platelet count,red blood cell content promoted as liver function classification enhanced（ P〈0. 05）; renal BUN,Cr,UA content decreased as liver function classification enhanced（ P〈0. 05）.Conclusion： Ultrasound can effectively evaluate the relationships between chronic liver cirrhosis liver function classification and changes of gallbladder wall thickness and anteroposterior diameter.
