振幅整合脑电图在缺氧缺血性脑病患者中的应用 被引量:9

摘要 目的:探讨振幅整合脑电图(aEEG)在评价心肺复苏(CPR)术后缺氧缺血性脑病(HIE)患者神经功能预后中的应用。方法:该翻E设置对照组和研究组。对照组用于建立参考值范围,对aEEG进行分级;研究组共计人组25例CPR—HIE患者,分别在入院24h、72h、1周时在床边进行aEEG监测,根据对照组研究进行aEEG分级、记录格拉斯哥一匹斯堡昏迷量表(G—PCS)评分和自主呼吸情况,分别在第1个月和第3个月时进行临床和脑功能评分(cerebral performalce categoryscale,CPC),分析各因素与神经功能预后之间的关系。结果:①研究组经过3个月的随访观察发现,复苏时间、年龄、性别与CPC评分之间无明显相关性,P均〉0.05;而aEEG、G—PCS、有无自主呼吸与CPC评分有明显相关性,均是P〈0.05,且呈线性相关;②多元线性回归分析显示aEEG评分与CPC之间关系最密切,通过aEEG评分可以较好地预测患者的神经功能预后。结论:aEEG等级、G—PCS以及自主呼吸对于CPR患者神经功能评分存在线性相关,可以根据三者的评分对CPRHIE患者的神经功能恢复进行预测。 Objective.. To explore the application of amplitude-integrated electroencephalography (aEEG) in evaluating the neurological function of patients with hypoxic-ischemic encephalopathy(HIE) with cardiopulmonary resuscitation(CPR). Methods:Two groups, the control group and the study group, were involved in the experiment,which was aimed at making up the reference value of aEEG of adults. The study group included 25 HIE patients with CPR. The 1 .tients's cerebral functions were monitored on the first day, the third day and the seventh day after they were admitted to ICU rooms of the hospital. The scores of aEEG,clasgow-pittsb urgh comg seore(G -PCS) and the clinical information of each patient were recorded. And there was follow-up observation within the first and third months, the score of cerebral performal cecategory(CPC) and the relationship between the CPC and the factors were analyzed. Resuits: First, after three-month follow-up observation, there was no obvious correlation between the scores of CPC and the gender,age and resuscitation time, P〉0.05. Contrastly, there was linear correlation be- tween CPC and the aEEG, G -PCS and respiration. Second, the multiple linear regression analysis showed rather close relationship between the aEEG and the CPC and the outcome of neurological function could be. Conclusion:There is linear correlations between aEEG, G -PCS and respiration with CPC, useful for neurological function with the scores of aEEG,G -PCS and respiration to patients with HIE after CPR.
