胸腔镜辅助下Nuss术治疗学龄期儿童与非学龄期患者漏斗胸对比

Comparison of Nuss surgery assisted by thoracoscope in the treatment of children with funnel chest between school age children and non-school age patients

目的总结胸腔镜辅助下两切口Nuss手术矫治学龄期儿童（年龄6—12岁）与非学龄期患者（年龄〉12岁）漏斗胸的临床经验。方法选取我科白2012年1月至2015年1月收治漏斗胸患者共67例（学龄期儿童35例，非学龄期患者32例），择期在胸腔镜辅助下行两切口Nuss术，观察手术时间、手术出血量、住院时间、并发症的发生、手术效果等相关指标。结果67例患者均顺利完成手术。非学龄期患者手术时间、出血量、住院时间、矫形板翻转数均明显高于学龄期儿童组。从矫形疗效上分析，学龄期儿童矫形效果明显好于非学龄期患者。结论胸腔镜辅助两切口Nuss术是治疗漏斗胸的重要术式。非学龄期患者胸廓的弹性和柔韧性较学龄期儿童差，学龄期儿童矫形效果更佳。提倡学龄期儿童尽早手术矫形漏斗胸。

Objective To summarize the clinical experience of Nuss surgery assisted by thoracoscope in the treatment of children with funnel chest between school age children （6 - 12 ） and non-school age patients （ 〉 12 ）. Methods 67 patients with funnel chest from January, 2012 to January, 2015 were selected, including 35 cases of school-age children and 32 cases of non-school age. All of them were given Nuss surgery under thoracoscope, and their operation time, bleeding volume during operation, hospital stay, complications and operation effect were ob- served and compared between the two groups. Results 67 cases were successfully completed surgery. Their operation time, amount of bleeding, duration of hospital stay, and turnover of orthopedic plate were significantly higher in the non-school age group than in school age group. The curative effect was better in the school age group than in the non-school age group. Conclusion Nuss surgery under thoraeoscope is an important method for the treatment of funnel chest. The elasticity and flexibility of the chest profile in non-school age patients are worse than those of the school age children, and the curative effect of school age children is better.
