细菌溶解产物联合糖皮质激素及白三烯调节剂治疗小儿哮喘的临床研究 被引量:20

摘要 目的评价细菌溶解产物(泛福舒)联合糖皮质激素以及白三烯调节剂治疗小儿哮喘的临床疗效。方法将83例轻度、中度的小儿支气管哮喘患者,随机分为对照组(n=42)和实验组(n=41)。对照组给予常用药物吸入型糖皮质激素(布地奈德)治疗,实验组在对照组的基础上再给予口服泛福舒以及白三烯调节剂治疗。比较两组患儿外周血中T细胞亚群和嗜酸性粒细胞水平、日夜间哮喘症状评分以及总有效控制率。结果治疗8周后,同对照组相比,实验组患儿外周血中CD+4、CD+4/CD+8细胞明显地升高(P<0.05),而CD+8水平显著下降(P<0.05),外周血中嗜酸性粒细胞显著降低(P<0.05)。两组患儿治疗前后日夜间哮喘症状评分均显著下降(P<0.05),且实验组下降幅度显著高于对照组(P<0.05),对照组与实验组临床疗效总有效率分别为76.19%和95.12%,差异具有统计学意义(P<0.05)。结论细菌溶解产物(泛福舒)联合糖皮质激素以及白三烯调节剂治疗小儿哮喘,能够明显改善患儿的免疫功能。 Objective To investigate the clinical efficacy of Broncho-Vaxom combined with corticosteroids and lcukotriene modifier in the treatment of children with asthma. Methods 78 children with mild, moderate bron- chial asthma were randomly divided into the control group (rt =42) and the experiment group (n =41 ). The control group received conventional drugs inhaled corticosteroid ( budesonide ) treatment, and the experiment group were treated with Broncho-Vaxom and leukotriene modifier on the basis of the control group. The data of T cell subsets and eosinophils level in peripheral blood, asthma symptom scores during daytime and night, and total effective control rate were compared between the two groups. Results 8 weeks after the treatment, the levels of CD4 and CD4+/CDs cells in peripheral blood increased more significantly in the experiment group than in the control group ( P 〈 0.05 ), but the level of CD8+ cells and eosinophils in peripheral blood decreased significantly ( P 〈 0. 05 ). The asthma symptom scores during daytime and night decreased significantly in the two groups ( P 〈 0. 05 ), and the decrease was more pronounced in the experiment group than in the control group ( P 〈 0. 05 ). The total clinical efficacy in the control group and the experiment group was 76. 19% and 95. 12% respectively ( P 〈 0. 05 ). Conclusion Broncho-Vaxom combined with corticosteroids and ]eukotriene modifier can significantly improve their immune function in the treatment of children with asthma.
