老年卒中患者鼻饲应用营养泵对卒中相关性肺炎发生的影响分析

Effect of nasal nutrition feeding pump for elderly stroke patients on stroke-related pneumonia

目的探讨老年卒中后患者,鼻饲应用营养泵对卒中相关性肺炎发生的影响。方法将70例卒中后需要鼻饲患者,随机分为对照组35例,治疗组35例。对照组给予常规间断鼻饲治疗,治疗组患者鼻饲给予持续应用营养泵,观察两组患者SAP发生率、营养状况变化及不良反应发生率。结果两组患者经不同的鼻饲方法治疗后,治疗组SAP发生率(17.1%)明显低于对照组(66.7%,P<0.05),治疗组血白蛋白、前白蛋白及血红蛋白优于对照组(t=2.155,t=2.782,t=2.037,P<0.05),治疗组胃潴留、腹泻、误吸、高血糖发生率(14.29%,17.14%,25.71%,17.14%)低于对照组(37.14%,40.00%,57.14%,45.72%,P<0.05)。结论对老年卒中后患者鼻饲应用营养泵与常规间断鼻饲相比较,能改善患者营养状况,胃潴留、腹泻、误吸、高血糖发生率低,减少SAP发生率,值得临床推广。

Objective To investigate the effect of of nasal nutrition feeding pump for elderly stroke patients on stroke-related pneumonia. Methods 70 stroke patients were randomly divided into the control group （ n = 35 ） and the treatment group （ n = 35）. The control group received conventional intermittent nasogastric treatment, and the treatment group were treated with nasal continuous application of nutrition pump. The incidence of SAP and the incidence of adverse reactions were observed between the two groups. Results The incidence of SAP was 17. 1% in the treatment group, which was significantly lower than that in the control group （66. 7%, P 〈 0. 05 ）. The level of serum albumin, prealbumin and hemoglobin was better in the treatment group than in the control group （t = 2. 155,t = 2. 782, t = 2. 037, P 〈 0. 05 ）. The incidence of gastric retention, diarrhea, aspiration, and hypoglycemia was 14.29%, 17.14%, 25.71% and 17.14% respectively in the treatment group, which were all lower than those in the control group （37.14%, 40. 00% , 57. 14%, 45.72% , P 〈 0. 05 ）. Conclusion Nasal nutrition feeding pump can improve the nutritional status of elderly stroke patients, and reduce the incidence of gastric retention, diarrhea, aspiration, and high blood sugar.
