摘要
目的 分析铜川市煤矿行业2007—2014年各期煤工尘肺发病以及晋期特点,为尘肺病防治工作提供科学依据。方法 对2007—2014年铜川市诊断报告的872例煤工尘肺新发病例与晋期病例进行分析研究。结果 872例尘肺病例中,新病例503例,新发病率Ⅰ、Ⅱ、Ⅲ期分别占93.84%、4.37%和1.79%。在职的659例尘肺患者中,Ⅰ、Ⅱ、Ⅲ期平均发病年龄分别为46.02±5.20、46.01±4.92、46.68±4.51;各期尘肺发病年龄均在40-50岁高发,平均发病年龄接近46.01-46.68岁。晋级病例369例,在职人员平均晋级间隔年限在4.88-6.37 a,离退人员平均晋级间隔年限在16.21-25.99 a,离退人员壹期直接晋级到叁期的占35.36%。结论 新发尘肺病例以壹期为主,在职人员平均发病年龄低,发病较早,晋级病例在职与离退人员差异大,离退人员晋级间隔年限长,壹期跳晋叁期比例大,健康监护不及时。
[Objective]To analyze the epidemic situation and stage-promotion characteristics of pneumoconiosis in coal industry in Tongchuan City from 2007-2014, provide the scientific basis for prevention and control of pneumoeoniosis.[Methods]872 new cases and stage-promotion cases of coal worker's pneumoeoniosis in Tongchuau City from 2007-2014 were analyzed. [Results]There were 503 new cases in 872 pneumoconiosis eases. Among the new cases, the incidence rate of stage I , stage II and stage III was 93.84%, 4.37% and 1.79%, respectively. Among 659 in-service pneumoeoniosis patients, the average onset age of stage I , stage II and stage III was respectively 46.02±5.20 years old, 46.01±4.92 years old and 46.68±4.51 years old. The incidence rates of different stages were highest in 40-50 year-old groups, and the average onset age was 46.01-46.68 years old. There were 369 stage-promotion eases. The average interval period of stage-promotion in in-service cases was 4.88-6.37 years, while that in retiree cases was 16.21-25.99 years, and retiree cases directly advanced from stage I to stage III accounted for 35.36%.[Conclusion]Most of new pneumoconiosis cases are in stage I . The average onset age of in-service patients is younger, and there is significant difference in stage-promotion cases between in-service population and retiree population. The retirees have longer interval period of stage-promotion, and retiree cases directly advanced from stage I to stage III account for great proportion. The health care should be improved further.
