天津市和平区口腔医护人员诊疗歧视态度调查

Investigation on discrimination attitude among doctors and nurses of stomatology in Heping District of Tianjin

目的 了解天津市和平区口腔医护人员诊疗歧视态度情况。方法 通过专门设计的调查问卷,以和平区17所口腔专科医院、综合医院口腔门诊的524名医护人员为调查对象,采取匿名填写的方式,进行基础资料、口腔诊疗歧视态度情况的信息收集。结果 收回有效问卷524份,其中医生278人（53.1%）,护士246人（46.9%）。口腔医护人员感知感染危险程度的平均得分为（23.12±2.65）分,诊疗歧视态度的平均得分为（8.40±2.03）分。多因素非条件logistic回归分析显示,口腔医护人员歧视拒诊患者的意愿主要受医院性质和级别、感知危险得分和歧视态度得分的影响,其OR（95%CI）分别为0.253（0.080-0.800）、3.633（1.741-7.581）、1.373（1.236-1.525）和4.922（3.666-6.608）。结论 口腔专科医院较综合医院口腔科,低级别医院较高级别医院的口腔医护人员更有歧视拒诊倾向,而感知危险和歧视态度调查得分高的受访者也更倾向于拒诊患者。应加强口腔医护人员关于感染防护知识及防护技能的培训,使其掌握标准操作规程及预防职业暴露,消除歧视与恐惧,以便更好地开展口腔诊疗活动。

[Objective]To know the status of discrimination attitude among doctors and nurses of stomatology in Heping District of Tianjin.[nethods]524 doctors and nurses were collected from 17 stomatology hospitals and stomatology department of general hospitals in Heping District of Tianjin. An anonymous questionnaire survey was conducted by the specially designed questionnaire, to collect the information of basic conditions and discrimination attitude.[ResultslThere were totally 524 effective questionnaires, including 278（53.1%） doctors and 246 nurses（46.9%）. The average scores of perception in infection risk degree were（23.12_＋2.65） points, and the average scores of discrimination attitude were（8.40_＋2.03） points. The multiple non-conditional logistic regression analysis showed that intentions to discrimination and rejection against patients were mainly influenced by hospital type and grade, scores of perception in infection risk degree and scores of discrimination attitude, which the OR value （95% CI）was 0.253 （0.080-0.800）, 3.633 （1.741-7.581）, 1.373 （1.236-1.525）and 4.922 （3.666-6.608）, respectively. [Conclusion]The doctors and nurses in stomatology hospitals and low-grade hospitals have higher intentions to discrimination and rejection against patients than those in stomatology department of general hospitals and higher -grade hospitals, and the respondents with higher scores of risk perception and discrimination attitude are prone to reject patients. It is necessary to strengthen the training of infection prevention knowledge and protection skills among doctors and nurses of stomatology, implement the standard practice and exposure prevention, and eliminate the discrimination and fear, to improve the quality of treatment.
