上海市闵行古美社区普查高血压肾损害情况及其与Rho激酶关系的研究 被引量:1

Study of Hypertensive Renal Damage in Shanghai Minhang District and its Relationship with Rho Kinase

摘要 目的:分析上海市闵行区成人高血压病患者中肾损害情况及其与Rho激酶的关系。方法:查阅上海市闵行区古美社区常住人口30岁以上人群健康档案,根据资料完整性共收集20 452例高血压患者。依据24 h尿微量白蛋白排泄率(UAER)分为正常白蛋白尿组(A组)、微量白蛋白尿组(B组)和临床蛋白尿组(C组),各组随机抽取100例,选取正常体检者50名作为对照,采用固相夹心酶联免疫吸附试验(ELISA)法测定各组血清Rho激酶1(ROCK-1)和TGF-β_1水平,分析ROCK-1和TGF-β_1在各组表达规律及与肾损害的相关性。结果:(1)在高血压患者中发生肾损害比例为20.62%;(2)与对照组比较,A、B、C组ROCK-1及TGF-β_1血清浓度均显著升高(P<0.05);且A、B、C组间ROCK-1和TGF-β_1血清浓度差异有统计学意义(P<0.05);(3)血清TGF-β_1与ROCK-1浓度与蛋白尿程度呈正相关(P<0.05);血清TGF-β_1与ROCK-1浓度之间呈正相关(r=0.761,P=0.038)。结论:高血压性肾损害24 h尿微量白蛋白排泄率程度与ROCK-1、TGF-β_1浓度呈正相关,Rho/ROCK信号通路可能参与了高血压肾损害的过程。 Objective: To analyze the hypertensive renal damage in Shanghai Minhang District and its relationship with Rho kinase.Methods: To obtain complete data from the health records of resident population in Minhang district,20 452 patients with hypertension were analyzed in this study.According to 24 hour urine trace albumin excretion rate( UAER),patients were divided into normal albuminuria group( group A),microalbuminuria group( group B) and clinical albuminuria group( group C),100 cases from each group were randomly selected,50 normal physical examination were selected as the control group.Using solid phase sandwich enzyme-linked immunosorbent assay( ELISA) method for determination of serum Rho kinase1( ROCK-1) and TGF-beta1 level,analysis the level of ROCK-1 and TGF-beta1 in each group and correlation with renal damage.Results:( 1) The proportion of renal damage in hypertensive patients was 20.62%;( 2) The level of ROCK-1 and TGF-β_1in group A、B、C were significantly higher than those in control group( P〈0.05),which were significantly different between group A、B、C( P〈0.05).( 3) Serum levels of TGF-β_1and ROCK-1 were positively correlated with the degree of proteinuria( P〈0.05).Serum levels of TGF-β_1and ROCK-1 were positively correlated( P = 0.038,r = 0.761).Conclusion: The level of urinary micro albumin excretion rate was positively correlated with the concentration of ROCK-1 and TGF-β_1in patients with hypertensive renal damage,and the Rho / ROCK signaling pathway may be involved in the process of renal damage of hypertension.
