下唇挡联合改良上颌斜面导板矫治下颌后缩的临床疗效观察

Observation on the clinical effect of mandibular lip bumper combined with improved maxillary inclined guide plate in the treatment of mandibular retrusion

目的分析应用下唇挡与改良上颌斜面导板联合矫治生长发育期下颌后缩错牙合的临床疗效。方法取2008年至2012年16例由于下唇肌及颏肌紧张导致下颌发育不足、下颌后缩的病例,均处于替牙晚期或恒牙早期。应用双期矫治,即先采用下唇挡经过8~10个月的I期矫治,解除下唇肌及颏肌紧张,再用固定矫治器结合改良上颌斜面导板矫治器进行Ⅱ期矫治。结果 16例患者治疗后下颌骨均明显向前生长,SNB角增大,ANB角明显减小,前牙覆牙合覆盖及磨牙关系基本正常,侧貌协调,矫治前后有显著差异。结论下唇挡结合改良上颌斜面导板矫治由下唇肌及颏肌紧张导致下颌后缩可取得良好效果,不仅可以解除下唇肌及颏肌紧张,还能刺激下颌骨前端骨质生长的作用。

Objective To analysis the clinical curative effect of mandibular lip bumper and improved maxillary inclined bite- plate in the treatment of growth and development period mandibular retrusion malocclusion.Methods Patients with mandibular deficiency and mandibular retrusion due to lower lip muscle mentalis muscle tension from 2008 to 2012 were contained in this clinical observation.The mandibular lip bumpers were used as phase I treatment for 8-10 months to relieve the muscle tension of lower lip and chin,then fixed appliance combined with improved maxillary inclined guide plate appliance as phaseⅡtreatment.Results After the treatment,the 16 patients grew forward obviously,SNB angle increased,ANB angle decreased.Overbite,overjet and molar relationship were basically normal,profile coordinated,they were significant different from these before treatment（ P 0.05）.Conclusions Effects are obvious when mandibular lip bumper is combined with improved maxillary inclined guide- plate in the treatment of patients with mandibular retrusion.The treatment can not only relieve muscle of lower lip and chin muscle tension,but also stimulate the growth of mandible front- end bone.
