子宫肌瘤采用单双极电器械腹腔镜下子宫次全切除术的临床分析

Clinical analysis on conducting subtotal hysterectomy by single/double pole electrical appliance under the guidance of laparoscope for the treatment of hysteromyoma

目的分析研究子宫肌瘤使用单双极电器械行腹腔镜下子宫次全切除术治疗的方法和效果,为临床提供依据。方法选取2013年1月至2014年12月子宫肌瘤患者资料86例实施回顾性分析,将86例患者随机分为两组,观察组43例,对照组43例,观察组患者采用单双极电器械行腹腔镜下子宫次全切除术进行治疗,对照组患者采用传统腹式子宫次全切除术进行治疗,对观察组和对照组患者的手术时间、出血量、尿管保留时间、肛门排气时间、镇痛泵使用时间及住院时间等进行记录和比较,将结果实施统计学分析。结果观察组和对照组患者手术时间对比不存在明显的差异(P>0.05),不具有统计学意义;观察组患者手术过程中出血量少,肠道恢复时间、尿管保留时间、手术后镇痛泵使用时间及住院时间明显少于对照组患者(P<0.05),具有统计学意义。结论采用单双极进行腹腔镜下子宫次全切除术是安全可靠的,与传统开腹手术比较患者的恢复时间快、出血量少、创伤小、患者住院时间短、术后并发症少,值得临床特别是基层推广使用。

Objective Analysis and research the method and effect of conducting subtotal hysterectomy by single / double pole electrical appliance under the guidance of laparoscope for the treatment of hysteromyoma,to provide the basis for clinical.Methods Retrospectively analysis the datas of 86 cases of patients with uterine fibroids,they received therapy during January 2013 and December 2014,and they were randomly divided into two groups,observation group 43 cases,control group 43 cases.Patients in observation group were treated by double pole electric appliance laparoscopic hysterectomy,while patients in control group were treated by traditional abdominal hysterectomy,the operation time,blood loss,urine retention time,anus exhaust time,analgesia pump use time and hospital stay were recorded and compared.Results The difference of operation time between observation group and control group was not statistically significant（ P 0.05）.The differences of bleeding,intestinal recovery time,urine tube retention time,postoperative analgesia pump use,anal exhaust time and hospitalization time between observation group and control group significantly less than the control group（ P 0.05）.Conclusions Single / double pole electric appliance laparoscopic subtotal hysterectomy is safe and reliable,compared with the traditional laparotomy it has the advantages as fast recovery time,less blood loss,small trauma,short in- hospital time,less postoperative complications,worthy of clinical promotion,especially in grassroots hospitals.
