耳穴贴压联合艾灸治疗绝经后骨量减少患者的临床疗效

Clinical effect of auricular point sticking combined with moxibustion on postmenopausal bone loss

目的通过前瞻性队列研究探讨耳穴贴压联合艾灸治疗对绝经后肝肾阴虚型骨量减少的临床疗效。方法将92例中医辨证为肝肾阴虚的绝经后骨量减少患者,随机分为试验组(30例)、雌激素(E_2)治疗对照组(31例)、空白组(31例),前2组分别采用耳穴联合艾灸治疗和口服激素治疗,空白对照组不接受特殊干预,研究期限为3月,比较3组间镇痛有效率、治疗前后疼痛强度评分变化、以及治疗前后骨密度(BMD)、E_2、血清骨钙素(sOC)、降钙素(CT)、甲状旁腺素(PTH)的水平。结果试验组和对照组的镇痛有效率均大于空白对照组,差异有统计学意义(P<0.05);试验组和对照组治疗后疼痛强度较治疗前均有减轻,差异有统计学意义(P<0.05),2组间比较差异无统计学意义(P>0.05);空白组研究前后疼痛强度差异无统计学意义(P>0.05)。试验组和对照组治疗前后BMD、E_2,、sOC、CT、PTH差异有统计学意义(P<0.05),2组间差异无统计意义(P>0.05);空白组治疗前后上述指标差异无统计学意义。结论耳穴贴压和艾灸穴位治疗能明显改善绝经后骨量减少患者疼痛症状及骨代谢指标,与对照组治疗效果相当。

Objective To observe the efficacy of auricular point sticking combined with moxibustion on postmenopause osteopenia deficiency of liver-yin and kidney-yin patients. Methods 92 patients were divided into three groups： experimental group（ 30 cases）,hormone therapy control group（ 31 cases）,blank control group（ 31 cases）. They were treated with auricular point sticking and moxibustion,oral hormone,no special intervention,respectively. The research lasted for 3 months. The analgesic efficiency,pain intensity scores and the levels of BMD,E_2,sOC,CT,PTH were detected before and after treatment and compared between three groups. Results The analgesic efficiency of the experimental group and hormone therapy control group was better than that of blank control group（ P 〈0. 05）; The intensity of pain relieved after treatment in both experimental group and hormone therapy control group. The levels of BMD,E_2,sOC,CT,PTH of the experimental group and hormone therapy control group were significantly increased after treatment（ P 〈0. 05）. Conclusions The effect of auricular point sticking combined with moxibustion for the postmenopause osteopenia patients is as good as hormone therapy. It can improve pain symptoms and bone metabolism obviously.
