摘要
目的探讨脂联素(APN)对脓毒症大鼠心肌损伤的保护作用。方法54只Wistar雄性大鼠随机分为假手术组(Sham)(n=18)、脓毒症组(cLP)(n:18)、脂联素组(cLP+APN)(n=18)。每组按照标本采集时间分为3h、6h、12h3个亚组。采用盲肠结扎穿孔法(CLP)制作脓毒症动物模型,Sham组仅行开关腹手术,CLP+APN组大鼠于CLP术后立即腹腔注射脂联素6mg/Kg。检测各组大鼠术后心肌TNF-α、IL-1,血清cTnI、尾静脉血糖水平,光镜下观察心肌组织病理形态变化。检测各组大鼠术前(0h)及术后3h、6h、12h尾静脉血糖水平。结果①与sham组比较,CLP及CLP+APN组大鼠术后3、6、12h血清cTnI、心肌TNF-α、IL-1表达水平均明显增高(P〈0.05);但CLP+APN组均明显低于CLP组(P〈0.05);②与Sham组比较,CLP组血糖水平3h达到高峰,6、12h明显下降(P〈0.05);CLP+APN组较CLP组均明显降低(P〈0.05),但与Sham组比较差异无统计学意义(P〉0.05);③CLP+APN组心肌病理损伤较CLP组有所减轻。结论脂联素可以显著减轻脓毒症时心肌损伤程度,对脓毒症大鼠的心肌损伤具有明显的保护作用。
Objective To explore the protective effects of Adiponectin (APN) on myocardial injury in sepsis rats. Methods A total of 54 Wistar male rats were randomly divided into three groups: sham operation group (n=18), sepsis group (CLP, n=18), and APN group(CLP+APN, n=18). Each group was divided into three subgroups: 3-hour, 6-hour, 12-hour groups according the specimen collection time. The septic animal models were created by adopting cecum ligation perforation (CLP). Sham group was only given for open-and-close abdominal operation, and CLP + APN group were immediately injected with APN 6 mg/Kg after CLP in the abdominal cavity. The dynamic changes of the indexes of myocardial injury were observed at various time points for each group, including cTnI in serum and tumor necrosis factor-or (TNF-α) and interleukin-1 (IL-1) in myocardium, blood sugar in tail vein. Then observation of the myocardial tissue pathology morphological changes was conducted under microscope as well as each group' s tail vein blood sugar levels prior to and 3 hours, 6 hours, 12 hours after the operation. Results 1. Serum concentration of cTnI, myocardium concentration of TNF-oL and IL-1 in CLP and CLP+APN group were significantly higher at point of 3-hour, 6-hour, 12-hour after CLP than the sham operation group (P〈 0.05); but they reflected low in CLP+APN group than CLP group (P〈 0.05)2. The level of blood sugar in tail vein increased significantly in CLP group at point of 3-hour after CLP but decreased at point of 6-hour, 12-hour in comparison with Sham group (P〈 0.05); the level of blood sugar of CLP+ APN group was lower than CLP group (P〈 0.05); changes of the blood sugar level did not demonstrate a significantly difference between CLP+APN group and Sham group (P〉 0.05). 3. The myocardial pathological damage of CLP+APN group reflected lesser serious level than CLP group. Conclusion APN is considered being effective in reducing sepsis myocardial damage, and carries an obvious protective effect for septic heart.
