归脾汤治疗特发性血小板减少性紫癜随机平行对照研究

Guipi Soup Treatment Idiopathic Thrombocytopenic Purpura Randomized Controlled Study

[目的]观察归脾汤治疗特发性血小板减少性紫癜疗效。[方法]使用随机平行对照方法,将70例住院患者按抛硬币法简单随机分为两组。对照组35例氨肽素,1g/次,3次/d;波尼松,10～20mg/次,3次/d。治疗组35例归脾汤（黄芪炙45g,阿胶、白术、人参、茯神、龙眼肉、酸枣仁各15g,白芍、山药、木香各10g,甘草炙6g,远志、当归各3g;肌肉出血加紫草10g,仙鹤草30g;月经淋漓不断加五味子10g,山萸肉15g;鼻出血、牙龈出血加藕节25g,五倍子10g）;水煎服,1剂/d,水煎400m L,早晚餐前1小时或餐后2小时口服,200m L/次。连续治疗28d为1疗程。观测临床症状、症状积分、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈18例,显效7例,有效6例,无效4例,总有效率88.57%。对照组痊愈10例,显效6例,有效7例,无效12例,总有效率65.71%。治疗组疗效优于对照组（P〈0.05）。症候积分两组均有改善（P〈0.01）,治疗组改善优于对照组（P〈0.01）。[结论]归脾汤治疗特发性血小板减少性紫癜疗效满意,无严重不良反应,值得推广。

[Objective]To observe the Guipi soup treatment of idiopathic thrombocytopenic purpura.[Methods]randomized,parallel controlled method,70 cases of hospitalized patients by a coin toss method is simple randomized into two groups.The control group of 35 patients ampeptidum,1g/time,3 times/d;prednisone,10 ～ 20mg/time,3 times/d.The treatment group 35 cases spleen soup（Huangqi 45 g,A＇jiao,Baizhu,Renshen,Fushen,L ongyanrou,Suanzaoren 15 g,Baishao,Shanyao,Muxiang 10 g,Gancao 6g,Yuanzhi,Danggui 3g;muscle bleeding plus Zicao 10 g,Xianhecao 30g;menstruation than dripping plus Wuweizi 10 g,Shanyurou 15g;nose bleeding,bleeding gums plus Oujie 25 g,Wubeizi 10g）;decoction,a/d,decoction 400 m L,early 1 hour before dinner or after dinner two hours of oral,200 m L/second;western medicine with the control group.28 d for a course of continuous treatment.Observation of clinical symptoms,symptom scores,adverse reactions.One course of treatment,to determine efficacy.[Results]The cure 18 cases,7 cases were markedly effective in 6 cases,4 cases,the total efficiency of 88.57%.The control group recovered 10 cases,effective in 6 cases,effective in 7 cases,12 cases,the total efficiency of 65.71%.Treatment group than the control group（P〈0.05）.Symptom scores improved in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.01）.[Conclusion]Spleen soup treatment of idiopathic thrombocytopenic purpura results were satisfactory,no serious adverse reactions,is worth promoting.
