逍遥散联合耳穴贴压治疗脑卒中后抑郁随机平行对照研究

Xiaoyaosan Joint after Auricular Acupressure Treatment Depression of Stroke Randomized Controlled Study

[目的]观察逍遥散联合耳穴贴压治疗脑卒中后抑郁疗效。[方法]使用随机平行对照方法,将88例住院患者按掷骰子法简单随机分为两组。脱水降低颅内压、扩张血管降血压、高压氧治疗、促进脑循环、抗凝血及溶血栓等治疗。对照组44例氟西汀,20mg/次,1次/d,20mg剂量无明显疗效,可逐渐增至60mg/d。治疗组44例逍遥散(北柴胡9g,当归、白芍、茯苓各15g,白术、郁金各12g,甘草3g),1剂/d,水煎300m L,早晚口服,150m L/次。耳穴贴压,选皮质下、心、肾、肝、神门、内分泌、耳背心,消毒耳穴或耳廓,镊子夹取粘有生王不留行籽的胶布,穴位贴压。连续治疗15d为1疗程。观测临床症状、HAMD评分、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组痊愈15例,显效17例,有效9例,无效3例,总有效率93.18%。对照组痊愈8例,显效12例,有效13例,无效11例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。HAMD评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]逍遥散联合耳穴贴压治疗脑卒中后抑郁疗效满意,无严重不良反应,值得推广。

[Objective]To observe Xiaoyaosan joint auricular acupressure treatment of depression after stroke effect.[Methods]Randomized parallel controlled method,88 cases of hospitalized patients were randomly divided into two groups craps method is simple.Dehydration reduce intracranial pressure,dilation of blood vessels lowering blood pressure,hyperbaric oxygen therapy,promote brain circulation,anticoagulant and thrombolytic therapy.Fluoxetine 44 cases in the control group,20mg/times,1 times/d,20 mg dose had no significant effect,can be gradually increased to 60mg/d.The treatment group 44 cases Xiaoyaosan（Beichaihu 9g,Danggui,Baishao,Fuling 15 g,Baizhu,Yujin 12 g,Gancao 3g）,1 ji/d,decoction 300 m L,sooner or later,oral,150 m L/times.Auricular acupressure,under election cortex,heart,kidney,liver,God doors,endocrine,ear vest,disinfection ear or ear,forceps sticky tape animate Vaccaria seeds,sticking points.15 d for a course of continuous treatment.Observation of clinical symptoms,HAMD score,adverse reactions.Continuous treatment for 3 courses,to determine efficacy.[Results]The cure 15 cases,17 cases markedly effective in 9 cases,3 cases,the total efficiency of 93.18%.The control group recovered eight cases,12 cases markedly effective in 13 cases,11 cases,the total efficiency of 75.00%.Treatment group than the control group（P〈0.05）.HAMD scores improved in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.01）.[Conclusion]Xiaoyaosan joint auricular acupressure treatment of post-stroke depression results were satisfactory,no serious adverse reactions,it is worth promoting.
