甘肃省武威市腹泻住院儿童腹泻病毒分布特征

Distribution of diarrhea virus among hospitalized children with diarrhea in Wuwei City, Gansu Province

目的分析2012—2013年甘肃省武威市腹泻住院儿童病例中病毒性腹泻病原体分布特征。方法采集2012年1月—2013年12月武威市5岁以下儿童确诊腹泻的住院病例信息及其粪便标本,采用RT-PCR、ELISA和PCR方法检测常见的病毒性腹泻病原体(星状病毒、轮状病毒、杯状病毒和肠道腺病毒)。结果 2012—2013年611例腹泻住院儿童病例中病毒性腹泻病原体检出率74.14%(453/611),其中星状病毒9.98%、肠道腺病毒13.91%、轮状病毒40.92%、杯状病毒40.43%,腹泻病毒混合感染检出率25.53%(156/611),男性儿童检出率74.38%(302/406)、女性儿童检出率73.66%(151/205);腹泻病毒检出率2013年75.17%(227/302),2012年为73.14%(226/309),混合感染检出率分别为25.57%(79/309)和25.50%(77/302);轮状病毒检测阳性的标本中P分型为P[6](20.80%,52/250)、P[4](14.40%,36/250)和G分型G3(17.60%,44/250)。结论 2012—2013年武威市住院儿童腹泻病例中病毒性腹泻的主要病原体是轮状病毒和杯状病毒,腹泻病毒的混合感染所占比例较高。

Objective To investigate the distribution of viral diarrheal pathogens in children with diarrhea in Wuwei city,Gansu Province. Methods Children with diarrhea hospitalized in Liangzhou hospital were selected for investigation and stool samples collection from Jan., 2012 to Dec., 2013. ELISA, PCR and RT-PCR methods were used to detect the viral diarrheal pathogens. Results Out of the 611 samples, 74.14%（453/611） were positive for viral diarrheal pathogens. The positive rate of human astrovirus（HAst V）, enteral adenovirus（EAd V）, human rotavirus（HRV） and human caliciviruses（Hu CVs）, was 13.91%（85/611）, 9.98%（61/611）, 40.92%（250/611） and 40.43%（247/611）, respectively. Of 453 cases positive for viral diarrheal pathogen, 25.53% were co-infected with 2 or more kinds of viral diarrheal pathogens. The positive rate of viral diarrheal pathogens was 73.66%（151/205） in female children, and 74.38%（302/406） in the male children. The positive rate of viral diarrheal pathogens in 2012 and 2013 were 73.14%（226/309） and 75.17%（227/302） respectively. And the proportion of co-infection was 25.57%（79/309） in 2012 and 25.50%（77/302） in 2013. The type of P6（20.80%, 52/250）,P4（14.40%, 36/250） or G3（17.60%, 44/250） were the most frequent types in children with viral diarrheal pathogen positive.Conclusions The major viral diarrheal pathogens of infants with diarrhea in Wuwei are HRV and Hu CVs. The proportion of co-infection of diarrheal virus among children under 5 years old is high.
