超声心动图评价紫杉醇对肿瘤患者左心室收缩功能的影响

摘要 目的探讨超声心动图组织运动二尖瓣环位移自动追踪技术(TMAD)评价紫杉醇对左心室收缩功能影响的价值。方法 52例接受紫杉醇治疗的肿瘤患者,应用超声心动图检测化疗前后左心室舒张末期内径(LVEDd)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)和左心室射血分数(LVEF),应用TMAD测量二尖瓣环6个位点的收缩期最大位移(Ds)。结果化疗后与化疗前相比,LVEDd、LVEDV、LVESV增加,差异均有统计学意义(均P<0.05),LVEF减少,但差异无统计学意义(P>0.05)。二尖瓣环6个位点Ds有不同程度减低,其中后间隔位点Ds减低显著(P<0.05)。结论TMAD有助于评价肿瘤患者紫杉醇化疗后左心室收缩功能的变化。 Objective To determine the value of speckle- tracking of mitral annular displacement (TMAD) in evaluating the changes of left ventricular systolic function after paclitaxel chemotherapy. Methods Left ventricular end- diastolic dimension (LVEDd), Left ventricular end- diastolic volume (LVEDV), Left ventricular end- systolic volume (LVESV), and Left ventricular ejection fraction (LVEF) were measured with conventional echocardiography, and the maximal displacement of six points of mitral annulus (Ds) was measured by speckle- tracking echocardiography in 52 cancer patients before and after chemotherapy. Results LVEDd, LVEDV, LVESV increased significantly after than before chemotherapy (P〈0.05). LVEF decreased insignificantly. The maximal displacement of six points of mitral annulus decreased accordingly after chemotherapy. The posterior- septal motion significantly decreased (P〈0.05). Conclusion TMAD may be benefit to evaluating the left ventricular systolic function change in cancer patients after paclitaxel chemotherapy.
