摘要
目的:拟通过追踪分析,探讨综合康复干预模式对睡眠呼吸暂停综合征(SAS)患者抑郁情绪的影响。方法:纳入2013年5月至2014年8月北京大学深圳医院干部保健科新诊断、未行持续气道正压通气(CPAP)治疗的中重度SAS患者50例为研究对象,随机分为对照组(n=21)与干预组(n=29),干预组给予综合康复治疗(干部保健科、营养科、心理科协同合作),3个月后观察研究对象的体重变化、CPAP治疗的依从性以及抑郁情绪的变化。结果:综合康复干预对中重度SAS患者的体重指数(BMI)及抑郁情绪有改善作用,BMI干预前29.2±3.5 vs干预后27.9±3.8,汉密尔顿抑郁量表评分干预前12.8±7.6 vs干预后7.6±4.7,P<0.05,但对SAS相关指标无显著影响(均P>0.05);3个月后干预组体重下降2%的患者比例为58.6%(17/29),对照组为28.6%,P<0.05;CPAP治疗比例为62.1%(18/29),对照组为33.3%,P<0.05。结论:综合康复干预可使中重度SAS患者的BMI有较为有效的降低,提高患者对CPAP治疗的依从性,改善患者的抑郁情绪。
Objective： A cohort study was designed to investigate the effect of combined intervention on depressive mood of patients with moderate and severe sleep apnea syndrome（ SAS）. Methods： From May 2013 to August 2014,50 patients with newly diagnosed,untreated moderate and severe SAS in our department were enrolled in the study. All patients were divided into control group（ n = 21） and combined intervention group（ n =29） randomly. Combined intervention involved personalized nutrition,exercise and psychiatric care. The variation of weight,tendency to CPAP treatment and improvement of depression were observed in 3 months. Results： The intervention decreased the BMI（ 29. 2 ± 3. 5 vs 27. 9 ± 3. 8,P 0. 05）,improved the depressive mood（ HAMD12. 8 ± 7. 6 vs 7. 6 ± 4. 7,P 0. 05）. The ratio of weight decreased 2% reached 58. 6%（ 17 /29） in combined intervention group,which was 28. 6%（ 6 /21） in control group. The application of CPAP was higher in combined intervention group（ 18 /29） than that in control group（ 7 /21）（ P 0. 05）. Conclusion： Combined intervention model can effectively decrease the weight,increase the use of CPAP and therefore improve the depressive mood in moderate and severe SAS.
